An employee turned the wizard on. It was on high amps and the sanding blocks flew off and hit a co-worker in the right hand.
There were no special conditions. EE was using a jack and the jack handle came back and hit EE in the mouth.

Member was hand pitching a piece of stone with a sledge hammer. This piece of stone was bigger than our normal sizes. A piece of metal came off the hammer and penetrated right forearm. Member immediately was transferred to hospital to get it removed. No permanent damage or impairment occurred.

Individual was climbing up the ladder of EE's loader to get in the cab. EE had both hands on the rails, but lost EE's footing and fell to the ground. EE hurt EE's right heel and right shoulder.

While cutting stone on the big saw, employee was using a pry bar to move the stone. While he was using maximum effort the stone moved - resistance going from maximum to nothing causing his arms to be thrust out in front of him while still holding the pry bar.
While stacking stones-palletizing tumbled stone Injury reached down to pick up a stone. While picking it up. Another stone fell out of the tumbler. His finger was caught between 2 stones that cut required 4 stitches.
Injury set his parking brakes and got off his forklift to instruct his fellow workers on. How to perform their job when finished. He turned rapadily and mistepped and his face collided with the master of the forklift. Resulting in 7 stitches-3 nose-4 cheek.
While picking up sawn stone with a forklift to place on a chopper employee got off the forklift to move a stone. A small stone fell off his head and cut his foot behind his steel top. Has some bruising and required stitches. Later got infected and is still being treated for infection.
The employee reported on the 16th that he believed he was bitten by a spider on 5/13/2008 and that it has gotten worse every day. He was sent to the Doctor. The Doctor told him he could return to work with restrictions, however the employee did not report back until 5/26/2008 with release from the Doctor.
While splitting stone in the quarry, EE removed the hammer to move to another drilled hole, lost his balance. Did not fall, but dropped hammer on his foot/toe.
While rotating rock on the conveyor the rock came in contact with the splitter and chips and dust were knocked off and irritated the eye. Employee reported eye problem on 2/22/08.
Safety glasses were being worn while chopping stone in extreme windy conditions. Some large dust got in his eye. Eye washing did not remove it all. Pupil was slightly scratched.
While EE was walking outside of large saw building. He tripped and twisted his ankle.
Employee was loading a conveyor belt that fed a tumbler. The stone was in a bucket, ground level, conveyor was waist high. He was wearng a back brace at the time.
EE was chopping irregular 4"" toffee in attempting to rotate stone for chopping finger was squeezed between the rock and the machine.

While drilling a hole under neathe a truck-the drill bound and slaned and twisted ee;s hand against the side wall-breaking his hand in 4 to 5 places.
EE said he was working on an engine and turned over and hurt his rib. Doctor says injury to chest wall.
While chopping double gray millends a large stone-28'x14""x4. He picked it up and threw it at an achward angle. He felt a sharp pain. Waited awhile-it went away-but later came back.
Back strain from lifting stone to palletize for skipping.
EE was lifting a millstone with a pry bar and strained his back.

The forklift operator allowed co-worker to get a ride on the forklift, which is against company rules. They were going from chopper #5 to #1 chopper for a break, when forklift hit a bump in the roadway and the rider inadvertently fell off of forklift and sprained his left ankle. Totally unacceptable; what were they thinking?
Employee was trying to measure the height of a stone when co-worker inadvertently moved controls on chopper without looking to make sure he was clear. His finger was resting on the roller pin to the conveyor and the conveyor rolled the stone back causing it to cut the tip of his finger off.
Miner was climbing onto Terex, (haul truck), when he said that he reached for the hand rail and either missed it or slipped off hand rail. He fell to the ground. He was then taken to the ER and had a dislocated shoulder which they put back into place.
He tripped over a small stone, slipped in the mud and fell on his right knee cap. At the present time it looks like a severe bruise.
Terex haulage truck operator washed the unit & then started to move to a parking area when the unit died. He exited the cab to check the engine compartment. When lifting the hood he slipped on the wet vehicle and slid to the ground (gravel road) landing on his knees.
Employee exited forklift to reposition forks. In doing so, he mashed his left finger between the forks and the mast.

TWO MEN PULLED A DRUM OFF THE AXLE & LAID IT ON THE GROUND. THEY PICKED UP THE DRUM & INJURED SAID IT FELT LIKE A CRAMP IN HIS STOMACH. DURING THE REST OF THE DAY, IT FELT LIKE A STOMACHACH. THE NEXT MORNING WHILE TAKING A SHOWER HE NOTICED A KNOT ABOVE HIS BELLY BUTTON. SAID IT WAS SORE & HURT. CEO SAID TO GO TO ER IF IT WORSENED.

EE WAS WORKING AT A BELT CONVEYOR, BEHIND A HYDRAULIC SPLITTER, WHEN A ROCK FELL FROM CONVEYOR ONTO HIS RT. FOOT, CAUSING SEVERE LACERATIONS TOTWO TOES. EE WAS NOT WEARING HIS STEEL TOE BOOTS AS IS REQUIRED BY THE MINE.

WHILE CLEARING A STORAGE BIN WITH A SKID STEER LOADER, THE OPERATOR STOPPED THE MACHINE LIFTED THE BUCKET APPROX 1/4 OF THE WAY UP. HE CLIMBED OVER THE BUCKET ONTO THE WALL OF BIN, THREW 2 STONES INTO THE BUCKET, HE THEN JUMPED TO THE FLOOR OF BIN & STARTED TO GET BACK INTO LOADER WHEN HIS FEET PRESSED THE LIFT CONTROL PEDAL CAUSING LIFT ARMS TO LOWER, PINNING OPERATOR
