An employee found a conveyor motor was turning but the gear reducer was not as the V-belt was slipping. The employee removed the guard and pulled the V-belt to add tension when the belt started and pulled ee's right index finger between the belt and pulley causing a laceration below the second knuckle resulting in 2 stitches and returning to work with no restrictions.

HE WAS PUSHING ON THE LOAD CAGE AS THE TOWMOTOR DRIVER ROCKED THE TOWMOTO R BACK AND FORTH. AS THE TOWMOTOR ROCKED FORWARD HIS RIGHT ARM BECAME PINCHED BETWEEN THE CAGE AND THE LAST PALLET OF BAGGED SAND, BREAKING HIS RIGHT WRIST.
EE WAS STEERING SCRAP CEMENT MIXER TO JUNK PILE. THE MIXER TRUCK WAS BEING PUSHED FROM BEHIND BY ANOTHER EE IN A 980G LOADER ON LEVEL GROUND AT LESS THAN 2 MPH. APPARENTLY THE RIGHT FRONT LOAD ER TIRE DIPPED INTO A SMALL SWALE ENOUGH THAT THE LOAD IN THE MIXER SHIFTED CAUSING THE MIXER TO ROLL OVER ON ITS SIDE.
