EMPLOYEE WAS DRESSING V-BELT AND HIS FINGER GOT CAUGHT BETWEEN BELT AND SHEAVE WHEN THE CONVEYORWAS TURNED ON BY ANOTHER EMPLOYEE. BOTH EMPLOYEES KNEW ABOUT LOCK-OUT PROCEDURES AND HAD THE PRO PER DEVICES TO DO SO CORRECTLY BUT FAILED TO DO SO.
