Employee notified supervisor that he had had surgery on his arm, and that the doctor told him it was job related. The doctor said it was nerve damage from laying arm on window seal of truck while driving.

Packaging palletizer #2 feed conveyor was coming apart at the splice. Employee asked forklift operator to get the lift cage with the forklift to assist him in the repair. While the employee was in cage he leaned forward and the cage and employee fell from forklift. The fall was from 4 feet. The employee sustained a head cut and bruised elbows.
