Loader operator was mucking out a shot when a rock fell from the roof and struck the top of the loader's ROPS, jostling the operator. EE suffered a broken rib and was hospitalized. There were no life threatening injuries.

Employee operating a yard loader backed into a triaxle truck. Employee waited days later before reporting injuries to supervisor and was assigned to restricted duty beginning 10/26/2009.
Employee was driving a tractor-trailer, loaded with fines, down into the pit, as he reached the intersection of the "New Ramp" and "4th Level Ramp" the tractor-trailer overturned into the bank. The employee was pinned in the overturned cab and had to be removed by emergency personnel. The employee sustained a fractured pelvis.
Employee complained of pain in back after driving haul truck. The injury was not reported to management for some time afterwards and the determination of being related to work took several weeks, during which time the employee lost 3 days making it reportable.
While watering roads, operator ascended hill towards the rock truck dump. Mid-way the engine of the water truck stalled, causing the water truck to roll backwards down the previously watered roadway. Operator lost control and the rear driver's side tires encountered the safety berm causing the truck to roll over onto it's side.
Employee felt a pinching sensation in his back and his leg started to go numb.
A loading truck was off of the loading road and had his truck stuck in a loading area.Our loader operator was unaware of the vehicle directly behind him and as he backed was unable too see truck and made direct contact.No damage to loader still waiting on quotes for truck.The only injury was a neck strain to loader operator.

EMPLOYEE WAS LOADING PIECES OF SCRAP METAL INTO BUCKET OF SKID LOADER WHEN A PIECE OF METAL CAME OVER THE TOP OF THE BUCKET. HE STUCK HIS HAND OUT TO PROTECT HIMSELF.
EMPLOYEE PULLED ON A CHAIN DISLODGING THE PULLEY, CAUSING IT TO SWING THE CHAIN, IT PULLED HIS FINGER BETWEEN THE CHAIN AND CONVEYOR STRUCTURE.
EMPLOYEE WAS OPERATING A 980G LOADER AND THROUGHOUT THE DAY NOTICED A PAIN IN HIS LOWER BACK. MANAGEMENT WAS NOT NOTIFIED UNTIL THE FOLLOWING WEEK.
EE FOLLOWING NORMAL WORK DUTIES OPERATING EUCLID FELT SHARP PAIN IN HIS SIDE AT END OF WORKDAY. PAIN PERSISTED INTO THE NEXT DAY. FOLLOW UP AT LOCAL EMERGENCY ROOM RESULTED IN DIAGNOSIS OF FRA CTURED RIB.
