Miner was switching railcars, drove a buggy between a car being pushed and another parked, and was struck, causing EE's leg to be entrapped between the dash and seat.
Miners working to clear out a clogged screw conveyor, miner didn't follow the company Lock out/tag out policy and caught their finger in the upstream screw causing an amputation of the left hand ring finger distal phalanges.
The employee used their right hand to wipe clean the underside of the trough, in doing so the employee's middle finger proceeded to enter through the hole and contact the rotating screw conveyor.
Miner was working in man lift when EE accidently switched the lift into high gear. The lift hit a large, hidden rock and jolted several times. The miner's lanyard shortened with each jolt and EE could not dislodge EE's foot from the dead man's switch. EE sprained EE's right ankle.
773 haul truck went over the edge of a 20' high stockpile. The truck driver suffered a minor laceration to the head and was taken to the hospital where EE was administered 6 staples to close the wound.
Employee sat the wagon down on the track, the motor revved up and took off uncontrollably and hit the car. Shuttle wagon maintenance repair, arrived on site and did an inspection/functions test and was unable to find anything wrong with the wagon.
Employee was driving man lift across track 3 crossing; man lift bounced EE up causing EE to strike EE's leg on the man lift tray under the controls. Employee suffered a break to tibia, fibula and bone was jammed up under kneecap.
Employee was crossing tracks in Shuttle wagon and brakes stop working. The shuttlewagon to hit tracks hard causing EE to bounce in seat, jamming EE's back.
Employee was driving tractor trailer on company property when trailer collapsed in the middle.
Employee was operating a lift truck in shipping area when EE struck a set of fixed rollers. Employee reported lower back pain and was escorted to local emergency room for observation.
Employee was traveling from the 740 up the incline. EE stopped, and set the parking brake, to tell the Doosan Operator that the heading was ready for to scale, when the parking brake failed. The machine rolled back approx. 50 feet and hit the rib.
The employee was inside a retractable cage on top of a bulk truck loading the truck. The truck moved forward without warning and pinned the employee's leg between the truck hatch and the retractable cage ladder.
Employee swung man-lift to bolt location hitting drill boom. Right hand was mashed between the man-lift and boom. One broken finger and one mashed finger.
An operator was operating a loader to help clean up one of the site roadways due to excessive rain over the weekend. The operator ran over a rock that caused the loader cabin to rock. This rocking motion resulted in a lower back strain/backache. The operator was seen by the local ER and placed on restricted duty.
While operating the scaling machine W/GS-2, jarring of the scaler cab had resulted in back strain/pain. The employee immediately reported this to his supervisor, requesting medical attention.
Employee was backing Scaler #1 from heading on 890 level with head turned to see behind. While backing up, scaler ran over small rock in roadway, scaler cab rocked, straining muscle in neck.
Employee was walking behind a forklift, as the forklift was backing up. Left rear wheel of lift partially ran over the employee's left heel. Back up alarm on lift was working at time of injury. Injured employee was taken to ER for evaluation and released back to work.
Miner driving R-40 Euclid Haul truck into south portal of Mine #4, bed was still partially up, bed struck roof of mine which is approximately 100 feet inside portal opening, casuing damage to front of truck bed. Minor back strain to employee.
Employee was operating mechanical scaler on 820 ft level. Attempting to remove loose rock, placed tip into rock, tip slipped out of the crack, causing boom to move forward, rocking the scaler cab back and forth. Sudden movement caused strain in back and shoulder area.
Employee was adjusting seat in Euclid haul truck, felt burning sensation in lower back. Went to doctor on 12/29, low back strain.
Employee was passenger in the powder truck. Truck was stopping at the mine office, truck operator stated throttle stuck and the truck struck a company pickup and the side of the mine office. No initial injuries, passenger requested medical treatment the next day. The powder truck was inspected and no defects were found.
Employee was in a manbasket attached to a forklift. The basket was 5 feet above the ground. While lowering the basket, the forks stuck momentarily, the chain continued to move, then the forks fell 3-1/2 feet stopping the basket 1-1/2 feet above the ground. The sudden stop casued the employee to strike his throat on the manbasket handrail.
Haul truck driver sitting in cab waiting to be loaded. Loader placed a large piece of marble in the bed, jolting the truck cab. The rocking motion injured the haul trucker operator's lower back.
Employee was adjusting the tracking on a newly installed conyeyor belt, right hand slipped and was caught between the belt and a small tension roller. Cut to the right pinky finger (four stitches) and abrasions to the back of the right hand.
EMPLOYEE WAS DRIVING 25 TON HAUL TRUCK UP GRADE ON HAUL ROAD, WAS HIT BY 35 TON HAUL TRUCK TRAVELING DOWN GRADE ON SAME HAUL ROAD. MINOR DAMAGE TO ONE TRUCK. HAUL ROAD ON SURFACE NEAR PLANT #5 . ROAD WAS WET AND VERY SLICK.
EE WAS LYING ON THE FLOOR, PARTIALLY UNDERNEATH THE CARDBOARD SLIPSHEET DISPENSER AT PLANT 4, ATTEMPTING CLEAN UP ACTIVITIES. HIS FEET WERE POSITIONED IN THE ADJACENT TRAFFIC AISLE. SAFETY SHOES PREVENTED MORE SEVERE INJURIES.
EE WAS STANDING NEXT TO PALLET LOADED WITH BAGGED PRODUCT. FORKLIFT ATTEMPTED TO PICK UP PALLET, PUSHED PALLET 4"-6" AND STRUCK EE'S LOWER RIGHT LEG. INJURY WAS MINOR, NO CUT, JUST SMALL BRUIS E, TAKEN TO DR AS A PRECAUTION.
EE WAS SWEEPING AROUND PALLET OF BAGS, STEPPED INTO PATH OF FORKLIFT. FORKLIFT DRIVER DID NOT SEE EE AND RAN OVER RT. FOOT, BACKUP ALARM WAS OPERATING. INJURED EE WAS WEARING STEEL TOE SHOE S AT THE TIME OF THE INCIDENT.
EMPLOYEE WAS DRIVING HAUL TRUCK #36 DOWN HAUL ROAD IN MINE, LOST CONTROL AND HIT WALL RESULTING IN MODERATE DAMAGE TO LEFT FRONT OF TRUCK AND A BRUISE TO EMPLOYEE'S HIP.
EMPLOYEE WAS CLIMBING ONTO TOP OF BULK TANK TRUCK IN ORDER TO LOAD. AS HE GOT TO TOP OF TANK THE DRIVER IMMEDIATELY MOVED TRUCK FORWARD BEFORE EMPLOYEE COULD HOOK UP FALL PROTECTION. EMPLOYEE GRABBED SAFETY CABLE WITH LEFT HAND TO PREVENT FALLING AND STRAINED SHOULDER.
LARGE ROCK HUNG IN FEEDER #3 RAYMOND MILL, KICKED FEEDER OFF. EE ATTEMPTED TO ROLL FEEDER DRIVE BACKWARDS BY HAND USING DRIVE BELTS. AS FEEDER ROLLED BACK AND ROCK CLEARED, DRIVE ROLLED FREELY CARRYING HAND INTO FEEDER DRIVE BELTS AND SHIEVE.
EMPLOYEE WAS TRAVELING TOO FAST FOR CONDITIONS, WITH A 980 LOADER, WITHOUT HIS SEAT BELT FASTENED, AND HIT A SERIES OF BUMPS CAUSING LOADER TO BOUNCE, THROWING HIM AGAINST ROOF CAB.
EE REACHED INTO MOVING SCREW CONVEYOR TO GET A PRODUCT SAMPLE AND THE MOVING SCREW FLIGHT CAUGHT THE END OF HIS RIGHT MIDDLE FINGER, RESULTING IN A MASHED FINGER.
