Employee was checked and released to full duty. 5/1 employee reported symptoms what was thought to be a personal medical. On 5/11 EE received restrictions may be linked to 4/26. Employee has been referred to a neurologist, results are forthcoming.

Employee ran off the road and into a ditch while driving

Employee was injured when the mine passenger van in which he was riding hit a rough spot on the road causing the injured passenger to hit his head on the van headliner.

After loading at the 112 shovel, Haul Truck 262 passed the pole bridge and went over a rough patch of road. The truck was traveling approximately 9 MPH when operator was bounced around, jarring his back.

EE WAS OPERATING THE TANKHOUSE STRIPPER CRANE HARVESTING CATHODE. WHEN THE EE PLUGGED CRANE, THE POWER TO THE CRANE TRIPPED OUT. EE TRIED TO RESET THE POWER, BUT COULD NOT. THE CRANE FORCEFULL Y HIT THE CRANE STOP ON THE NORTH END OF THE BUILDING. THIS RESULTED IN DAMAGE TO THE CRANE STOPS AND JARRED THE OPERATOR INSIDE THE CRANE. EE SUSTAINED A LOWER BACK STRAIN.
