Employees were placing 24"" HDPE pipe, when the pipe suddenly shifted and rolled on top of one of the workers. He was treated and stabilized at the scene. He was subsequently transported to a hospital, where he later passed away.

Employee turned to get grab a PVC pipe and when they twisted, they felt a pop and discomfort in their lower back.

Employee had an initial incident on 12/12 which resulted in a lost time incident and the employee had surgery. On 8/14/24 the employee was working normal routine duties when the surgery knee had a numb feeling. The surgeon has advised there is inflation in the area of the surgery and advised the employee not work to rest the knee.
Employee was getting a 6"" pipe from the pipe rack when the pipe began to roll off, the employee attempted to catch the pipe and employee's left pointer finger was smashed between the rack and the pipe. The employee was taken to the local hospital for further treatment.

While operating the crash end of the stripping machine a bent blank went over the stepper on one side and became jammed. The employee hit the e-stop on the side to prevent the machine from dragging the stuck blank. The employee tried to dislodge the blank by hand and when it was freed from the jam EE's hand got caught in a pinch point. The employees left middle finger was crushed.
The employee was holding a cathode bar with right hand and using wood harvesting tool with left hand to pry the cathode free from the anode insulator. As the cathode broke free from insulator, the cathode dropped, pinching employee's finger between the cathode bar and the cell bus bar.
Employees were Staging a 40' Crane Rail against a wall, Injured Employee was struck by the Rail and fractured EE's lower right fibula down by the ankle.

Employee injured left shoulder when EE grabbed the railing while exiting a water truck due to worn step grading.
Employee climbing down a ladder felt a pop in right wrist. Employee was given restrictions, due to restrictions employee is unable to do normal job.
Driver was delivering Copper Concentrate to FMI Rail hopper. It was drizzling rain. EE opened the tailgate of the trailer to make sure load was completely delivered. EE slipped and grabbed onto the trailer to keep from falling. EE's middle left finger got slammed in the tailgate resulting in 9 stitches and fracture.
Employee attempted to step over a driscoll pipe that was connected to a pump and slipped hitting EE's chin on the pump.

EE was using a 5lb scraper when EE felt a sharp pain in lower right side of back as EE was making a combination of downward and off to the side motions. EE tried to work the pain out but it wouldn't get better.
777 haul truck operator was jarred around in the cab due to excavator loading the HT made contact with headache rack. Employee was checked and released to full duty. 5/1 employee reported symptoms what was thought to be a personal medical. On 5/11 EE received restrictions may be linked to 4/26. Employee has been referred to a neurologist, results are forthcoming.
Employee was reconnecting pothead to a backup generator. The pothead arched and employee received 2nd degree burns to middle two fingers on the left hand.
Dust fell from above when climbing ladder with attached retractable lanyard
Employee was assigned to stripping starter sheets. A starter sheet was sticking to the blank. Lifting up on the sheet to get a more leverage to pull down again the blank also lifted causing it to come off the hooks. The blank weighing about 50 lbs fell on the top part of employee's foot. The impact caused soreness to the upper portion of EE's left foot. Restricted for 1-day.

While the employee was conducting construction quality assurance for a project at the mine, he walked forward a few feet down the slope and stepped onto articulated concrete block mat (which had some gravel on the surface) and he slipped falling backward and landing on his left shoulder, causing a contusion and ruptured rotator cuff.
Removing angle iron holding wooden form in place, the angle iron broke loose pinching EE's finger between the angle iron and a shelf of the wooden form.
On February 24, 2014, employee was diagnosed with carpal tunnel from repetitive motion of stripping cathode starter sheets at the Miami EW Tankhouse. Per employee the symptoms began 12/7/2013.

Employee was moving a copper starter sheet weighing approximately 12 Lbs. from a moving vertical rack to a horizontal conveyance system and felt pain in the right mid back area. Strain resulted in lost time of one day.
Dozer operator rolled #640 dozer while cleaning narrow road. Dozer went thru the berm at the crest on the 2950 level and came to rest upright on the crest of the 2900 level. Employee was seen and had a full release to work. When seen by company doctor on 9/3/2013, PT was ordered making the incident reportable. There was no lost time and no restrictions.
While shifting forms for a concrete pour, the employee whipped his eye to remove sweat from his brow. While whipping the eye brow he accidently pushed a small flake of metal into his eye without realizing it. It began to irritate him so he informed a foreman who called health and safety. He was transfered to a medical facility to have the flake removed.
Employee was on the stripper line and working to get a starter sheet unstuck. His hand slipped causing his gloved hand to contact the sharp edge of the sheet causing a laceration of his left pinky finger.

Worker was driving the pin for a nuclear gauge with a small sledgehammer, and during the course of the swing, the sledgehammer struck the edge and deflected, causing a shoulder strain.
Employee was cutting radiator hose on 272 HT with a utility knife. The knife slipped and caused a cut on the right index finger.
WHILE WORKING ON A CONCRETE PAD OUTDOORS USING ALL-SOLVE SOLVENT TO REMOVE PROTECTIVE COATING (COSMOLEEN) FROM MATING SURFACE ON THE FINAL DRIVE OF A CAT 793 TO INSTALL REAR TIRES. EMPLOYEE SAID THAT THE WIND PICKED UP CAUSING SOLVENT MIST TO GET IN BETWEEN HIS SAFETY GLASSES AND HIS RIGHT EYE. EMPLOYEE WAITED 2 WEEKS TO REPORT INJURY.
Maintenance mechanics were removing rollers from a track dozer. Mechanic positioned himself approx 12' away while other struck the roller with 16lb sledge. When the roller was struck, a shard of metal broke off and struck the mechanic who had stepped back, striking his right cheek.
Maintenance employee was cleaning a V-seal using a cleaning agent. The employees left hand caught fire as employee was working and the employee sustained 1st and 2nd degree burns on his left hand and forearm.

Electrician performing maintenance on 112 shovel was preparing to leave the shovel. He opened the spring latch door to the man-lift and used his hip to hold the spring gate open as he backed into the basket. Below a welder was preparing to board the shovel. The welder started lowering the basket just as the electrician stepped back. The electrician fell to the ground.
While working on a metal plate at the weld shop a small piece of metal came off and got stuck in employee's left eye. Employee flushed eye but was unable to remove the fragment.
Employee was carrying two slings and a clevis to the boom truck. He put the slings on the truck and dropped the clevis on the ground. When he bent over to pick up the clevis, and began picking it up, he heard a pop in his back and fell face down. He crawled over to the truck when his supervisor came over and asked how he was.
Employee was about,10-20 feet away from the 111 shovel as crew was working on replacing teeth on the dipper. One of the crew was using an 8-12 pound sledge hammer on the tooth in order to remove it. The hammer struck metal and a piece flew off striking injured employee in the left elbow.
After loading at the 112 shovel, Haul Truck 262 passed the pole bridge and went over a rough patch of road. The truck was traveling approximately 9 MPH when operator was bounced around, jarring his back.

Employee was working under wheel well of haul truck when he was struck by a falling piece of mud.
Employee was washing 429 van when the hose he was using got stuck. When the employee flipped the hose to get it loose, he struck the van with his elbow causing a contusion.
EE was jackhammering concrete at the tankhouse when a piece he was working on broke off. The jackhammer fell on the EE's foot. The x-rays showed that the EE's toe next to his big toe was fractured. The EE was restricted to 3 days of light duty.
Employee was working at the lower quickfill. He was removing bad order cable. He bunched up the cable with his green strap and pulled the slack out of the strap. When he finished, he felt pain in his right hand
Employee was moving copper bars out of the way at cell 18. The last bar slipped out of his hand, smashing his right index finger between the copper bar and the bus bar.
Employee bent over to pick up pvc pipe and sprinkler assemblies. He stood up and felt back pain.

Employee was removing a rock from under a 16"" hdpe pipeline with a shovel. Ground had been softened by recent rains. While applying force on the shovel, it rotated, twisting the employee's leg. The twisting force caused the employees kneecap to rotate approximately 45 degrees coming to rest on the ouside of his left leg.

EMPLOYEE WAS REMOVING A HYDRAULIC STEERING PUMP FROM HAUL TRUCK #249. THE EMPLOYEE WAS USING A TELESCOPE STAND TO LOWER THE PUMP FROM POSITION. THE PUMP WAS RESTING IN A SADDLE IN TOP OF THE S TAND. AS THEY LOWERED THE PUMP DOWN, THE PUMP FELL OUT OF THE SADDLE AND STRUCK THE EMPLOYEE. THE EMPLOYEE SUSTAINED A LACERATION TO HIS HAND REQUIRING TWO SUTURES.
WHILE TROUBLESHOOTING MOTOR STARTER EMPLOYEE THOUGHT THAT HE HAD RACKED OUT STARTER BEFORE PLACING HIS LOCK ON LOCK OUT. THE RACK DEVICE WAS STILL IN THE ""ON"" POSITION BUT THE DOOR WAS ABLE TO BE OPENED, THINKING THAT THE STARTER WAS LOCKED OUT THE EMPLOYEE TOUCHED THE INDICATOR ON THE MAIN FUSE, RECIEVING A 2ND DEGREE BURN ON HIS RIGHT INDEX FINGER, RIGHT KNEE AND LEFT HAND.
EE WAS HELPING TROUBLE SHOOT ON #3 BOILER WHEN HE WENT TO REACH FOR THE AUTOMATIC WATER LEVEL BLOWDOWN VALVE HIS FOREARM CAME IN CONTACT WITH A HOT STEAM LINE THE EE SUSTAINED A 2ND DEGREE BUR N.
EE SMASHED HIS MIDDLE FINGER ON HIS RIGHT HAND WHILE PULLING CATHODE BAR FROM STORAGE STANDARD SHEET. THE EE SMASHED HIS FINGER BETWEEN CATHOD BAR AND I-BEAM CAUSING A LACERATION AND FRACTURE REQUIRING THIRTEEN SUTURES.
EE'S HAND BECAME PINNED BETWEEN A BIT SUB AND PUP STEEL WHILE LOADING A BIT SUB. HIS KNUCKLE WAS CUT. ON 2-19-01 HE WENT TO THE DR. DUE TO AN INFECTION INHIS KNUCKLE.
EE WAS DISASSEMBLING TWO SECTIONS OF SCAFFOLDINGWHEN HE WENT TO DISCONNECT ONE OF THE TOP BRACESHE & THE SCAFFOLDING FELL TO THE GROUND WITH THESCAFFOLDING LANDING ON HIM CAUSING A CONTUSION T O HIS LEFT LEG. HE WAS INITIALLY TREATED CONSERVATIVELY. HE WAS REFERRED TO A SPECIALIST. HE HADSURGERY ON 8/1/01.

EMPLOYEE SUSTAINED A LACERATION TO HIS FINGER ON HIS LEFT HAND WHILE PULLING ON A BAR. THE BAR BROKE FREE SMASHING HIS FINGER BETWEEN THE STACK OF BARS AND THE HE WAS PULLIGN ON. THE LACERAT ION REQUIRED SUTURES.

HE WAS TREATED AT HOSPITAL FOR STITCHES, BRUISES AND BROKEN NOSE.
