While changing forklift operators during suspended load operation, one of the operators inadvertently pressed the park break release switch while grabbing a hold of the grab handle when descending - causing the forklift to roll and the load to shift. Injured employee was caught in between pipe flanges - resulting to an injury to their wrist and thumb.

Operator of loaded Haul Truck traveling to dumping assignment reached to turn on their flashers to go around a rock spill in the roadway and felt a sharp pain in their right shoulder. NOTE: Shift started at 6:30 PM on 05/23/22; incident occurred on 05/24/22 at 12:30 AM.

Operator was descending down a ramp with a fully loaded Bulk truck at about 15 miles per hour following a water truck. Operator had both the retarder and jack brakes applied but started to slide and over corrected and slowly turned the truck over.
A rock was dislodge by a pickup causing it to fall into a crusher where miners were working. 1 miner was stuck on the head while wearing a hard hat. The miner was knocked unconscious. Miner received first aid for the incident, and returned to work.
On the above date and time trainee and trainer on 209 was backing under the bucket at shovel 11. Neither the trainee or trainer heard a shovel horn, and the bucket tripped dropping the muck in the bed. The trainee was still backing up, and put the truck in neutral and quickly set the park brake.
While being loaded at the loader and the knuckles on the bucket hit the top side of the haul truck. Employee did not feel like they needed medical attention at that time. On 8/23 employee complained that their back was worse and employee was taken to the clinic the morning of 8/24/20.

At approximately 6:35pm the operator of HT152 was getting loaded at SH23, during the backing up process the operator of HT152 came in contact with the dipper of SH23 causing the dipper to trip releasing the material into the back of the truck. Incident is under further investigation.
While operating haul truck from N3 to BD4000 dump, operator reported hitting bumps in the road and also that the bed of haul truck slammed down during 2 loads. This caused them to jar and experience lower back pain.
while sitting in buddy seat of haul truck, a large rock went over the teeth of bucket and landed on the front right corner of the canopy causing employee to jar hurting EE's back
While being loaded in Haul truck, a large boulder was in the first bucket. The truck bed was not padded and the hard load jolted the employee causing mid back pain

While reinstalling fuel tank, employee inadvertently placed hand on forklift fork supporting tank. Forklift operator shifted forks for positioning and pinched employee's right finger between tank and fork
While dumping a tilting trash hopper using a forklift, employee lifted hopper with forklift onto a rolloff and pinched left middle finger between fork and hopper base.
Driver swerved to avoid deer in road. Driver was speeding. Tractor/trailer rolled on its side and was towed. Driver was treated and released from hospital morning of accident with minor cut to head, bruised hip and sore muscles.

The employee was riding in a transport bus to EE's job assignment when the bus hit a bump in the haul road. The employee then reported to EE's supervisor that EE was feeling some back pain after the bus hit the bump. The employee was taken to the clinic to be examined and after the examination, the physician pulled EE off work until EE is able to see a specialist.
