The CLG truck driver was attempting to dump a bulk load of ash at Giant Cement. The unit turned over while at the 3rd stage level. The driver was evaluated and issued prescription medication.

The employee was driving a utility cart back to the third bank silos. Visibilty was reduced due to weather conditions. The employee hit the vertical stand of a truck wash. The employee was injured due to the impact.

As the employee was riding on a rail car, EE observed a forward car to begin derailing. Upon seeing the forward car derail; the employee jumped off of the car EE was riding. EE injured knee upon landing causing a work restriction.

The employee was driving a utility vehicle through the facility. The utility vehicle impacted a large rock at the corner of an intersection and tipped over causing injury to the employee's middle finger (left hand), ring finger (left hand), and left hip.

Driver was delivering sand to Giant Cement Company. Driver was dumping his end dump at the sand pile when he heard a pop and the raised bed started to tilt towards the drivers' side. Equipment ended up overturning on drivers' side. Ground conditions at site were hard and level. Appears Hyd Cyl. pin backed out of connection.

Plant Maintenance Mechanic was inspecting conveyor belt. Apparently while inspecting the belt, his glove on his right hand was pulled into the pinch point between one of the troughing idlers and the belt. He suffered a laceration on the right index finger, which require sutures to close the wound.

A Plant electrician locked up the brakes on a Kawasaki 4 wheel motor cycle while turning a corner. The employee lost control of the vehicle and suffered contusions to his right knee and head. Further medical evaluation on 5/28/08 indicated that the employee suffered a possible torn minisus in his RT knee. He has been placed on modified duty effective 5/28/08.

Employee was operating man lift from bucket. Boom was in the travel position. When employee crossed rail tracks, he apparently was bounced upward while still tied to the bucket anchor point. He fell back down striking his forehead on the control panel. He suffered a gash on his forehead which required some stiches.

HAUL UNIT PASSING ANOTHER, OPERATOR LOST CONTROL AND ROLLED OVER. IT WAS DETERMINED THAT THE CAUSE WAS OPERATOR ERROR. SPEED, OVER STEERING AND THE BOWEL IN THE HIGH POSITION WERE THE CAUSE.

EE WAS WORKING ON SCAFFOLD INSIDE KILN. AS A LOAD OF BRICK WAS BEING TAKNE INTO THE KILN ON A FORKLIFT, THE DRIVER STOPPED JUST SHORT OF THE PLATFORM THE SCAFFOLD WAS SITTING ON. THE WEIGHT OF THE PALLET OF BRICK CAUSED THE FORKLIFT TO SLIDE ABOUT 6" INTO THE PLATFORM. THIS EXTRA FORWARD MOVEMENT ALLOWED THE PALLET OF BRICK TO STRIKE THE EE IN THE GROIN.
