During a pause on the line, he leaned forward & lifted his left foot up & set it on a slow moving foot high conveyor belt. His foot moved along an inch or so & slipped down between that belt & the next one in the line, which was not operational. Upon removal he went to the ER-nothing broken just swelling & missed one day on ice at home. (No damage to steel toe boot either.)

A BAG FELL FROM PICKING ARM ONTO THE CONVEYOR. EMPLOYEE WENT TO PICK UP THE EMPTY BAG AND THE ROLLERS GRABBED THE BAG AND AT THE SAME TIME, PULLED LEFT HAND BETWEEN ROLLERS AND BELT.
