Miner was attempting to clear an obstructed troughing roller with a small hammer. The miner was standing on the cross support of the screen deck #1, clearing a roller on the 1st under screen belt. Miner slipped and EE's right arm fell into the back side of the guarding, EE's right hand came in contact with the belting and roller resulting in a cut to EE's right middle finger.

Operator noticed a situation with the brake system on truck. As EE approached lower storage area EE was unable to stop. Operator diverted truck against stored material, contacting stored boulders, overturning dump bed and stopping.
WHILE CHANGING BUCKET ON BOBCAT EMPLOYEE LOCKED THE LEVER IN PLACE AND BEFORE HE COULD REMOVE HIS HAND THE OPERATOR PULLED THE BUCKET BACK SMASHING HAND.
