EE WAS THE LOADER OPERATOR SAW THE FINISH MILL RECALIM HOPPER WAS STOPPED UP, SO HE CLIMBED UP AND REACHED INSIDE THE HOPPER WHILE THE BELT WAS STILL RUNNING THE TRY AND MOVE THE LARGE CHUNKS OF ROCK. THIS CAUSED THE CHUNKS TO BREAK QUICKLY CATCHING HIS HAND AND WRIST BEFORE HE COULD PULL THEM OUT, RESULTING IN A MAJOR LACERATION TO HIS LEFT HAND.

WHILE EMPLOYEE WAS WORKING AROUND GYPSUM/CLINKERBELT AREA, HE GOT GYPSUM DUST PARTICLES IN HIS RIGHT EYE. EMPLOYEE WAS WEARING SAFETY GLASSES, HARD HAT AND STEEL TOED BOOTS AT THE TIME OF THE INCIDENT. EMPLOYEE WAS TAKEN TO OPTOMETRIST AND A PARTICLE WAS REMOVED FROM HIS EYE. PARTICLE WAS IMBEDDED.
