THE INJURED EE WAS MOVING THE MINER CABLE AND WATER LINE WHICH WAS BETWEEN THE RFM AND THE RIB. THE MBC BACKED UP AND THE EE'S LEG WAS PINCHED BETWEEN THE DOLLY AND THE RIB.
THE EE WAS OPERATING A 3 WHEELER. THE MACHINE HIT A ROCK CAUSIG THE FRONT WHEEL TO TURN RADICALLY WHICH IN TURN CAUSED THE STEERING WHEEL TO SPIN STRIKING THE EE ON THE RIGHT HAND. HE BEGAN LOSING TIME ON 6/9/97.
EMP WAS OPERATING A SCOOP WHEN HE CRUSHED HIS LEFT INDEX FINGER BETWEEN THE CAB AND THE FRAME OFSCOOP.
THE EE WAS CLEANING BELT WHEN THE SHOVEL HE WAS USING BECAME CAUGHT BETWEEN THE BELT & ROLLER JERKING HIS RIGHT ARM. THE INJURED EE WORKED THRU 6/3/97.
EE REPORTEDLY WAS OPERATING SHUTTLE CAR AND HIT A BUMP IN THE RUNWAY.
EE REPORTEDLY RECEIVED A JARRING BUMP TO BACK WHILE TRAMMING THE MOBILE BRIDGE CONVEYOR.
ON 12-30-96 EMPLOYEE REPORTED THAT HE STRUCK A TRACK DOOR WITH THE MANTRIP HE WAS OPERATING AND STRUCK HIS RIGHT SIDE ON THE DOOR ITSELF. HE SUFFERED 2 CRACKED RIBS.

EE REPORTED PAIN AND SWELLING IN LEFT KNEE FROM NORMAL WORK DUTIES ON MAY 29 BEGAN MEDICAL TREATMENT ON 6-10-96. NO LOST TIME .
EE WAS RIDING ON MANTRIP SKID.
EMPLOYEE REPORTS THAT HE IS SUFFERING WITH PAIN FROM HIS RIGHT HIP TO HIS RIGHT HEEL. HE REPORTS THAT HE HAS HAD SYMPTOMS SINCE OCT 1995 AND FEELS THAT THIS CONDITION IS RELATED TO HIS NORMAL SHUTTLE CAR OPERATOR DUTIES AT THE MINE. HE STARTED MISSING WORK ON 1-9-96, THE DAY HE REPORTED THIS CLAIM.
EE HIT BUMP IN SHUTTLE CAR ROADWAY AND BUMPED HIS HEAD ON THE CANOPY OF THE SHUTTLE CAR CAUSING PAIN IN NECK.
EMPLOYEE WAS STANDING NEAR A BOX THAT WAS BEING LOADED UP BY A BATTERY SCOOP. HE WAS HOLDING A CABLE OUT OF THE WAY AND THE BUCKET ON THE SCOOP CAME DOWN ON TOP OF HIS LEFT FOOT, FRACTURING TH REE TOES.
EMPLOYEE REPORTEDLY TWISTED HEAD TO LOOK BACK. FELT PAIN. PLEASE NOTE: MEDICAL TREATMENT ONLY.
EMPLOYEE STARTED TO TRAM A MANTRIP. HE APPARENTLY FORGOT TO DISENGAGE THE DRAG. WHEN HE TRAMMED THE MANTRIP, THE DRAG DERAILED THE MANTRIP CAUSING HIM TO HIT FACE ON THE FRAME.
RUNNING EIMCO, FOOT SLIPPED, STUCK BETWEEN PLATE AND ROCK.

LEVELING MUCK PILE OFF AFTER BALST WITH AIR-POWERED EIMCO. WHILE RUNNIGN EIMOCO IN A FORWARD DIRECTION MACHINE DROPPED DOWN AND EMPLOYEE WENT TO BACK UP WHEN MACHINE WENT BACKWADS. EMPLOYEE S LIPPED AND FELL FORWARD AND LEFT FOOT WENT UNDER SDIE PLATE AND MACHINE ROLLED FORWARD AND PINCHED ANKLE.
EE WAS STANDING IN TRACK ENTRY NEAR AILOCK DOOR WHEN A SCOOP STRUCK THE WOODEN DOOR LATCH. A PIECE OF THE BROKEN LATCH STRUCK THE EE ALONGSIDE THE FACE.
EE WAS OPERATING AN EIMCO BATTERY SCOOP WHEN HE GOT HIS LEFT ARM CAUGHT BETWEEN THE SCOOP SEAT AND THE RIB WHERE A RUN THRU CANVAS WAS INSTALLED. IT CAUSED HIS WATCH TO BE TORN OFF OF HIS ARM. THE WATCH CAUSED A LACERATION WHEN IT CAME OFF OF HIS ARM.
WHILE ATTEMPTING TO UNCOUPLE TWO MANTRIPS EMPLOYEE PINCHED HIS FINGER BETWEEN THE UNCOUPLING DEVICED & THE MANTRIP HE RECEIVED 3 STITCHES IN HIS LITTLE FINGER ON HIS LEFT HAND
EMPLOYEE WAS IN THE PROCESS OF BACKING HIS SHUTTLE CAR UP SOME WHEN HE STRUCK THE RIB CORNER CAUSING THE EMPLOYEE TO STRIKE HIS HEAD ON THE SHUTTLE CAR COMPARTMENT HE RECEIVED 1 STITCH IN HIS FOREHEAD
THE EE REPORTED THAT HIS BACK STIFFENED UPON REACHING THE WORKING SECTION.
EMPLOYEE WAS OPERATING IN SHUTTLE CAR AND RUNWAY WAS ROUGH, EMPLOYEE BOUNCED IN HIS SEAT AND STRAINED BACK.
EMPLOYEE REPORTS THAT WHILE SCOOPING UP FEEDER AREA, HE INJURED HIS LEFT ELBOW. HE PULLED ON FEEDER REMOTE START PULLCHORD SNAPPED. WHEN THE PULL CHORD SNAPPED, HE BUMPED HIS LEFT ELBOW ON THE SCOOP FRAME.
EMPLOYEE WAS OEPRATINGA SCOOP IN A-R BUTT NAD HWILE DRIVING ALONG THE SUPPLY ROAD, THE SCOOP DROPPEDINTO HOLES CAUSING HIS LOWER BACK TO BE JARRED. EMPLOYEE REPORTED THIS INCIDENT TO THE EMPLO YEE ON 6-14-94. EMPLOYEE STARTED MISSING WORK ON 6-20-94.
EE WAS OPERATING UNIHAULER TO CONTINUOUS MINER, BUMPED UNIHAULER INTO THE END OF MINER TAIL AND JARRED BACK.
WHILE EMPLOYEE WAS ADJUSTING CONVEYOR BELT TRACKING A PIECE OF LUMP COAL CAME AGAINST A COUNTERWEIGHT FOR A BELT SCRAPER EMPLOYEE REACHED TO PUSH LUMP AWAY FROM THE COUNTERWEIGHT IN DOING SO T HE LUMP COAL SPUN & CAUGHT HIS LEFT RING FINGER BETWEEN THE LUMP COAL & THE SCRAPER COUNTERWEIGHT
EMPLOYEE WAS HELPING MOVE A POWER BOX INTO A X-CUT FROM THE TRACK ENTRY USING A CABLE AND SHIEVE ARRANGEMENT ATTACHED TO A 15 TON LOCOMOTIVE. AS TENSION INCREASED ON THE ARRANGEMENT, A CHAIN HOLDING THE CABLE GRIP BROKE, THAT WAS ATTACHED TO THE LOCOMOTIVE. THE CABLE GRIP FLEW APPROXIMATELY 45' AND STRUCK THE EMPLOYEE ON THE RIGHT FOOT.
REPORTEDLY THE INJURED WAS PLASTERING A STOPPING IN A-4 SECTION IN X-CUT 3-2. HE WAS ON THE NO.3 SIDE OF WALL. IT IS REPORTED THAT WHEN ANOTHER EMPLOYEE STARTED UP A BATTERY SCOOP ON THE NO.2 SIDE OF THE WALL, THAT IT TOOK OFF IN HIGH TRAM THROUGH THE STOPPING AND STRUCK THE INJURED EMPLOYEE ON THE OTHER SIDE OF THE STOPPING. ALSO, BLOCK FROM THE WALL STRUCK THE EMPLOYEE. HE WAS KN
EMPLOYEE WAS TRAMMING SHUTTLE CAR AND JARRED HIMSELF, CAUSING 2 CAPS ON TEETH LOOSE.

EMPLOYEE WAS OPERATING A 15 TON MOTOR ALONE IN 7 NORTH TRACK, WHEN HE STRUCK HIS HEAD ON A 60 LB RAIL THAT WAS USED TO SUPPORT THE ROOF. FELT PAIN IN HIS NECK AND RIGHT SHOULDER.
EMPLOYEE AS TRAMMING A SCOOP ON #4 ROOM OF A-3 ROOMS. WHILE TRAMMING, SCOOP BOUNCED FROM HITTING UNEVEN BOTTOM CAUSING HIM TO STRIKE HIS HEAD AGAINST THE CANOPY.
EMPL WAS OPERATING SHUTTLE CAR-WHILE TRAMMING TO FACE,THE CANOPY OF HIS CAR HIT THE FIRE EXTINGUTUISHER HANGING ALONG THE HAULAGE CAUSING IT TO DISCHARGE AND SPRAYED FACE AND EYES.
EMPLOYEE WAS GETTING READY TO MAKE BELT SPLICE.BELT TENSION RELEASED AND KNOCKED EMPLOYEE AGAINST TAKEUP DOLLEY.
EMPLOYEE WAS OBSERVING SLACK IN CABLE ALONG MBC NO. 1 UNIT WHEN THE MBC CRAWLER PAD RAN ATOP HIS FEET WHILE IN THE A-1 BUTT WORKING SECTION.
