EMPLOYEE WAS TURNING A PICKUP TRUCK AROUND ON THE SURFACE WHEN HE BACKED INTO A DRAINAGE DITCH TIPPING THE TRUCK ON IT'S SIDE. THIS CAUSED HIM TO SUSTAIN A STRAINED MUSCLE AND A CHIPPED TOOTH . SAW DR. ON 10-31-95.
EE WAS CROSSING UNDER THE BC-3 BELTLINE AT A CORVERED WALKWAY. THE 3 1/2 SCOOP MOVE FORWARD AND THE TIMBER PINCHED THE INJURED MINER PELVIS IN CAB. DR RELEASED TO WORK, NO TREATMENT PAIN SUBSIDED THEN RETURNED RETURN VISIT TO DR AND LOST TIME ON 7-20-94.

MANTRIP STRUCK BACK OF HAULER
MANTRIP CAME AROUND 90 DEGREE TURN UNDERGROUND AND STRUCK INTO THE BACK OF A SLOW MOVING SHIELD HAULER.WENT TO DR ON 04-30-92.
INJURED WAS HAULING MUCK INTO A RAMCAR, WHEN HE RAN OVER A LARGE ROCK IN THE HAUL ROAD, INJURED BOUNCED UP AND JAMMED HIS HEAD INTO THE CAB, INJURING HIS NECK.
AT THE END OF THE SHIFT, EE REPORTED BACK PAIN, HE HAD BEEN HAULING CONCRETE IN A RAM CAR ALL NIGHT BUT DOES NOT KNOW WHAT HAPPENED. HIS BACK WAS INJURED IN EARLY 1990 IN A OFF THE INJURY, HIS MISSED NUMBEROUS DAYS OF WORN.
INJURED WAS KNEELING BETWEEN TOP AND BOTTOM BELT HELPING INSTALL TOP IDLER ANOTHER PART OF THE SAME CREW HOOKED ONTO BELT WITH SCOOP TO PULL ENDS TOGETHER AND CONNECT BELT. AS THE BELT WAS PUL LED INJURED WAS MOVED IN TO X FRAME.

AFTER INSTALLING A SCRAPER ON A-2 HEAD ROLLER THE BELT WAS UNLOCKED AND STARTED FOR TESTING INJURED NOTICED SOME VIBRATION IN THE HEAD OF THE SCRAPER AND REACHED INTO THE GUARD TO FEEL HOW MUC H THERE WAS AS HE FELT THE SCRAPER A SPLICE HIT IT BOUNCING IT DOWN AND PINCHING HIS HAND AGAINST THE STOP PLATE
INJURED WAS RIDING ON SMALL TRACTOR WHEN OPERATOR ASKED HIM TO HOLD STEERING WHEEL WHILE OPERATOR PUT ON HIS GLOVES. TRACTOR HIT RUT IN ROAD CAUSING STEERING WHEEL TO SPIN & TWISTING INJURED HAND.
AS CREW GOING I3 MINE ON MANTRIP AT 1ST CORNER IN MINE TRIP OPER APPLIED BREAKS AND THEY DIDNT WORK THE PIN CONNECTS THE BRAKE PEDAL TO THE MASTER CYLINDER HAD FALLEN OUT.OPER STEERED TRIP INT O RIB TO SLOW I7 DOWN INJUREDS HEAD STRUCK MOTOR COVER.
