While operating shuttle car the victim ran thru a pothole causing the car to bounce and felt pain in his lower back.
THE VICTIM WAS TRAVELING UNDER THE HUNG MINER CABLE WHEN HIS SHUTTLE CAR CAME INTO CONTACT WITH CABLE TEARING IT DOWN AND CUTTING HIM BEHIND THE LEFT EAR.

THE EDGE OF THE UNIT STRUCK HIS RIGHT FOOT BEHIND THE HARD TOE RESULTING IN 4 FRACTURED TOES. ON 9/17/00 THE FOURTH TOE AND THE SMALL TOE WERE AMPUTATED DUE TO THE INJURY.
