Employee stated that he was driving a tandem coal truck hauling coal to the prep plant. As he was starting down a grade, he down shifted to a lower gear. He stated that it came out of gear & truck started to gain speed. As he proceeded downgrade, he applied brakes to slow the truck down but as he entered a curve, the truck turned over onto its right side causing multiple injuries.

Carrier bearing that holds drive line (shaft) broke allowing drive line (shaft) to move rupturing air line that goes to the brakes causing brakes to fail. At this time the truck (International Truck #15) begin rolling backwards. Employee jumped out of the truck landing on his back injuring shoulder (right) & his back.

Employee stated that he was delivering parts to the job site in the parts truck. The truck hit a pothole in the haul road causing the employee to be jolted in the truck. He said he felt pain in lower back area and down his right leg.

Employee stated that he had delivered parts to the job. He was coming down the road from the job when his parts truck hit a pot hole in the road. The truck jarred him causing low back pain. Reportable on 7-31-06 when lost time was incurred.

EE STATED THAT HE WAS ASSISTING IN SETTING AN ENGINE BACK INTO A DOZER. HE HAD HIS HAND ON THESPREADER CHAIN WHICH HELD THE ENGINE ONTO THE BOOM OF THE TRUCK. AS HE WAS GUIDING THE ENGINE IN T WHICH CAUGHT AND TWISTED THE EE HAND CAUSING FRACTURE TO LEFT THUMB.

AS HE WAS HAULING LOADS OF REFUSE OVER AN ALLEDGED RUTTED OUT ROAD, HE DEVELOPED PAIN IN HIS BACK AND NECK AR EA WHICH PROGRESSIVELY GOT WORSE. NOTIFIED FOREMAN ON 6/12/02 AND THEN WAS REPORTABLE DUE TO LOST TIME INCURRED ON 6/17/02.

EE WAS OPERATING A REFUSE HAULER ON THE IMPOUNDMENT AREA. AFTER HAVING DUMPED A LOAD OF REFUSE, HE WAS LEAVING THE FILL AREA ON AN UPHILL GRADE WITH THE BED RAISED FOR TRACTION. AS HE TOPPED T HE GRADE, THE HAULER JARRED HIM, JERKING HIM TO ONE SIDE & STRAINING HIS RIGHT SIDE. LOST TIME BEGAN 1/25/01.

EE STATED THAT HE WAS ATTEMPTING TO REMOVE A ROOF BOLT CAUGHT AT THE HEAD ROLLER ON THE RAW COAL CONVEYOR BELT. THE BELT WAS TURNED OFF BUT HAD NOT COMPLETELY STOPPED. EE GRABBED HOLE OF ROOF BOLT AND THE BOLT JERKED HIS HAND AGAINST THE HEAD ROLLER SIDE CAUSING LACERATION TO TWO FINGERS. REPORTABLE DUE TO REQUIRING SUTURES.
