Employee was standing in #16 track spur waiting to chain off ballast cars. Once the cars stopped, the employee began to connect the clevis to a car when the cars moved pinching the index and middle finger on right hand.

Employee was tramming a scoop when the canopy caught a waterline coupler. This caused the waterline to break, and the end of the waterline struck the employee in the head.

The employee was using a forklift to load pallets of cinder block into the scoop bucket. Unknown to the operator some blocks had come loose and had fallen in the roadway. As ee trammed the forklift ee ran over one of these cinder blocks and ee felt a pain in back from the jarring action. (Note: Received information that doctor had taken employee off work on 02/15/2017).

While tramming the shuttle car to the miner EE hit the panic bar as the shuttle car passed an open break. The brakes locked up on the shuttle car bringing it to an immediate stop. The sudden stop caused EE to jam his right index finger into the shuttle car controls, spraining his finger. He continued to work until 8/20/15 & is off taking physical therapy for his finger.
While employee and his crew were sitting in a mantrip, another mantrip came down the Alma Main Line track and did not see their mantrip, striking their mantrip from the rear.
While hauling supplies, he ran over a rock & was thrown into the top of the scoop, striking his right shoulder. Went to get x-rays, never missed any scheduled shifts. Worked normal duties until April 18, 2011. At this time he brought me a doctor's excuse stating he would have to be off work & get surgery on his right shoulder. Became reportable at that time.
While getting out of the scoop deck, EE got his ankle tangled up in a piece of line curtain which was being used to cover the seat. As he got out he twisted his ankle.
The employee was operating a Fairchild scoop from the operator's deck installing a dual 250, 42" belt drive. When the individual proceeded to move the drive assembly around a corner the injured was hit in the mouth by a chain being used to secure the drive.
EE was assisting coworkers unloading a section of stage loader off the rail car. When the scoop winch was engaged the chain clevis connected to the scoop cable winch broke allowing the winch cable to fly back striking the EE on the left shoulder, neck & face. Both the Federal (MSHA) & State (WVOMHST) were notified in a timely manner. EE was treated & released from the ER.
EE & co-worker were loading trash out of a scoop bucket onto trash sled located in the track. The scoop operator assumed all the trash was out of the bucket & started the scoop to move away from the trash sled & track. When he started retracting the ejector blade the injured EE's lt thumb sustained a laceration & fracture which later resulted in partial amputation of lt thumb.
While riding manbus to job site, manbus hit a dip along track, bounced injured ee's head into canopy of manbus. This pushed head downwards between shoulders. He used his right hand to cushion the fall injuring, hand, neck and head.
Employee was shoveling between the top and bottom belts when the shovel got caught between the top belt and the top of the top belt roller. The shovel flipped around and struck the employee on the right forearm.
Employee was riding #26 Jeep. The Jeep started to back up & employee hit his head on the 73' belt guard.

EE was resting his hand on the scoop blade, as the operator withdrew the blade, the EE's hand was caught between the back of the bucket and blade.
He was climbing up on end of loader and he felt his little finger hurting on his left hand.
Employee was seated inside mantrip waiting to go up hill on surface track. A second mantrip exited the mine on the same track and struck the first trip. Injury not reported as lost time until 3-1-06.
Employee was hauling dirt out for installation of rail road bed - going up hill with load of dirt to dump point - road at top of hill - left side of bank gave away - cab of truck went over the berm causing the cab & bed to roll down embankment.
Driver got truck out of gear going down hill, could not stop. Traveled to Mingo Logan property & turned over. Driver refused treatment on site. Later went to emergency room, treated and released to return to work.

An outby laborer received a laceration to his right foot when two mantrips collided, at a switch on the mine yard.
The motor operated by the injured employee had a broken coupling pin and mechanical problems preventing it from independent operation. It was chained to another motor pulling it. The coupling was being held in place by a jack bar. The pull motor stopped causing the coupling to make contact and pinch the injured's left hand between the bar and motor.
Park brake leaked off manbus causing it to run away striking rib when it derailed throwing ee around inside bus.
WHILE REMOVING CURB BLOCKS FROM UNDERNEATH SHUTTLE CAR EE MOVED TO SIDE AND SAID SHUTTLE CAR WAS TRAMMED BY ANOTHER EE AND CURB BLOCK FLEW UP STRIKING EE INTHE JAW & CHEEK AREA CAUSING FACIAL FRACTURES.
AFTER ATTACHING TWO CHAINS, FROM AN OVERHEAD HOIST, TO R 14 CM15 CONTINUOUS MINER, THE VICTIM'S LEFT THUMB BECAME CAUGHT BETWEEN THE TWO CHAINS WHEN TENSION WAS APPLIED.
THE MANTRIP BRINGING THE CREW TO THE SURFACE, DERVED, CAUSING THE VEHICLE TO DROP ONTO THE RAILS, THE EE WAS STRUCK INT HE BACK WITH A HAMMER HE WAS CARRYING IN HIS BELT.
WHILE OPERATING A SHUTTLE CAR, THE CAR STRUCK A LARGE ROCK IN THE ROADWAY, JARRING THE VICTIM.
SHUTTLE CAR BUMPED LOWER BY. THE SHUTTLE CAR STARTED SPINNING AND SLIDE BACKWARDS.
OPERATING MINER AND WAS TRAPPED BETWEEN BOOM OF MINER AND SHUTTLE CAR AFTER LOADING SHUTTLE CAR CAUSING COMPOUND FRACTURE OF RT LEG.
EMPLOYEE WAS RUN OFF THE ROAD BY 18 WHEELER COALTRUCK AND EMPLOYEE RAN INTO THE DITCH. EMPLOYEE WAS DRIVING A FORD F-250 TRUCK AND WAS PATROLINGTHE PROPERTY.
A LONG AIRDOX SCOOP WAS PULLING A FAIRCHILD SCOOP THROUGH A SEVERE SEAM DEPOSIT. A HOOK ON THE 3/8" CHAIN, BEING USED FOR PULLING, BROKE CAUSING THE CHAIN TO STRIKE THE OPERATOR OF THE SCOOP B EING PULLED.
OPERATING BUGGY-TRAMMING UP ENTRY, AND RAN OVER SOMETHING, CAUSING OPERATOR TOBE THROWN INTO CANOPY (HURTING NECK).
FELL ASLEEP OR FAINTED FROM SICKNESS DROVE CAT TRUCK INTO DITCH DRAIN LINE.
EE WAS DRIVING BACK TO LAB AFTER PICKING UP SAMPLES, WHEN HE BLACKED OUT AND CRASHED INTO ROCK WALL ALONG HAUL ROAD, INJURING HIS HEAD, HANDS AND KNEE.
WHILE OPERATING A SHUTTLE CAR, THE EE HIT A HOLEIN THE ROADWAY RESULTING IN A LOWER BACK STRAIN.
