Employees were using a scoop to move a roof bolter when one of the employees accidentally got his left arm between the scoop and the bolter resulting in a fracture to both bones in his left forearm.

Employee was inured on 11/06/12 when he stepped in front of a mantrip that struck his lower leg. He was later diagnosed on 11/28/2012 as having a chip in the tibula of his left leg, he began loosing time on 11/28/2012.

Employee was moving a battery powered personnel carrier when the vehicle struck the coal rib causing the employee to strike his chin against the machine. This resulted in a laceration that required 6 stitches.

Employee was operating a lo-trac scooping gob from a crosscut prior to the incident. While there were no witnesses and the employee cannot recall what happened, we believe his head came in contact with the roof or a 4 inch plastic water line.

EE was in the loader, another EE was directing him what to do. One EE told the other EE to push the dozer track over and gave him the motions to go ahead and do it. When it rolled over it landed on the EE's leg.

Employee was operating a rubber tired personnel vehicle when the front tire hit a hole in the road causing the steering wheel to spin around striking the back of his hand. This resulted in a small fracture to one of the bones in his hand.

Driver lost control of tractor trailer when engine failed and he lost air pressure after restarting engine. Trailer went into ditch. Tractor rolled onto haul road. Driver visited ER on 10/08 and was treated for strained lumbar. Prescribed 2 days off work.

Using a Lo-Trac to assist in hanging an airlock door. The employee was trying to steady the door by placing his left hand on the top of the door. The Lo-Trac raised the door and caught the employee's left ring finger between the roof and the top of the airlock door.

A roller was stuck on the 1 east belt at break #6. The EE attempted to free the roller without locking out or turning off the belt. His hand slipped and caught his lower right arm (forearm) between the belt and the roller. The Dr. took him off work on 3/7/06.

Driver had 2 previous verbal warnings concerning speeding. Turned over truck was found across tracks at bottom of "A" side lying on its driver's side. Transmission was in neutral, Jake turned off, engine still running, brakes still hot to touch, 1 hr 45 min later. Driver had attempted to shift gears going downhill & got the truck out of gear. Results, runaway truck.

He heard a noise at the #2 north belt drive and was trying to locate the source. The omega coupling located between the drive motor and the speed reducer came apart into four sections. A hard piece of plastic struck the employee on his left hand fracturing two bones in his hand.

TRUCK WAS DROVE INTO DITCH AFTER LEAVING SEVEN FEET OF SKID MARKS AND TRAVELING FORTY FT AFTER SKID WITH NO SIGN OF BRAKING INTO DITCH. TRUCK WAS FOUND IN NEUTRAL AND EE BRAKE TURNED OFF. NO DAMAGE WAS DONE TO TRUCK.

LOADER BOOM HIT HAULER BED WHILE LOAIDNG HAULER. THIS CAUSED THE HAULER OPERATOR TO BE JERKED SIDEWAYS WHILE INT HE OPERATORS SEAT HE COMPLAINED OF BACK PAIN AND WAS SEEN BY A DR. DR SAID POSS IBLE PULLED MUSCLE AND RETURNED HIM TO WORK. ON JUNE 13TH HE RETURNED TO THE DR MRI BEEN OF WORK SINCE JUNE 14TH.

HE OPERATED A LOADER ON 2-6-02, LOADER TIRE HIT A POTHOLE IN THE ROAD CAUSING HIS BACK TO BECOME MILDLY PAINFUL. HE OPERATED THE LOADER ON 2-8 AND 2-9. ON 2-9 HE WENT TO A LOCAL EMERBENCY ROOM WHERE HE WAS DIAGNOSED WITH A HERNIA OF A VERTEBRAE DISK.

EMPLOYEE STATED HE GOT HIS LEFT FOOT CAUGHT BETWEEN THE MOBLIE BRIDGE CARRIER AND THE TAILPIECE. HE DID NOT THINK THE FOOT WAS INJURED, HOWEVER THAT NIGHT THE FOOT STARTED TO SWELL. HE WENT TO THE DOCTOR ON 7/10/01 AND WAS DIAGNOSED WITH A CONTUSION OF HIS LEFT FOOT.

EMPLOYEE WAS OPERATING MANTRIP. THEY DID NOT REMOVE TIMBER FROM THE BACK OF THE VEHICLE. THE ROADWAY WAS MUDDY AND ROUGH. THE VIBRATION OF THE VEHICLE CAUSED THE TIMBERS IN THE BACK OF THE MAN TRIP TO SHIFT. THE END OF ONE TIMBER CAME IN CONTACT WITH THE RIB. THIS CAUSED THE OPPOSITE END OF THE TIMBER TO SWING AROUND, STRIKING HIM ON SIDE OF HIS FACE, JUST BELOW THE RIM OF HIS HARD

EMPLOYEE WAS INSTALLING SPILL CONTAINMENT SKIRTSON 2 WEST DUMP. THE DRIVE 2ND BELT WAS TAGGED & LOCKED OUT. OTHER EMPLOYEES WERE WORKING ON THE OTHER END OF THE BELT & THEY RELEASED THE PRESSU RE ON THE BELT TAKEUP. THE INJURED EMPLOYEE WAS ON THE BELT WORKING ON THE SKIRT WHEN THE BELT TAKEUP WAS RELEASED. WHEN THE BELT MOVED A FEW FEET IT CAUSED HIM TO HURT HIS BACK.

EMPLOYEE THOUGHT THE SECTION BELT HAD BEEN LOCKED OUT SO HE STARTED TO CROSS THE BELT. THE BELT CREW STARTED THE BELT TO LINE UP A SPLICE. THE BELT MOVED AND DRAGGED THE EMPLOYEE THROUGH A 12" CLEARANCE BETWEEN THE BELT AND THE BELT DOLLY.
