Employee had stopped at airlock doors, just inside portal, when a second mantrip entered mine and struck first mantrip resulting in a strain to employee in 1st mantrip. Bright sunlight outside had not allowed eyes of 2nd mantrip operator to adjust.

Showing all 22Operating a Hitachi 1700 Rock Truck traveling up an incline to the dump. As the truck approached the top section of the ramp, a large rock shifted causing the truck's front wheels to come off the ground and then back down, jarring the member. He returned to work, then on 4/1/2011 he was taken off work until 4/11/2011.
Going down hill on New Road. Ran over something, tore out plug in bottom of fuel tank & lost all the fuel out. Motor died out, lost brakes. Truck was checked at the scene. Was driver's fault.
Came around corner on 4-wheeler, bumped timber, timber fell onto hand causing injury. Taken to ER for treatment, stitches used on wound. Mine employee re-set timber.
Truck dropped off the road & went in the ditch.
Operating SC, went under RB cable; flat rock on top of SC was pulled by RB cable, sliding under canopy into lap of employee. Taken to E.R. for treatment. RTW 10-17-07. 11-5-07 returned to doctor due to pain; removed from work for 2 weeks.
At end of track on section, man-trip slid on wet track (top sweating), employee holding onto bar behind seat, man-trip bumped flat car causing box with SCSRs to slide. SCSR box struck hand causing injury.
Riding in man-trip holding onto cage. Trip stopped finger struck by loose battery cover causing injury. Went to ER at end of shift, stitches inserted as percaution. Return to work 3/15/06.
Front wheel of rubber tired personel carrier struck coal rib causing the steering wheel to spin rapidly. The assist steering knob struck injureds left arm causing a fracture to the radius bone.
EE WAS CLEANING OLD BELT ENTRY WITH A SCOOP TO SET A TAIL PIECE. AS EE WAS SCOOPING AN OLD BELT RAIL CAME INTO THE OPERATORS COMPARTMENT AND STRUCK EE IN THE RIGHT SIDE. EE WAS TAKEN TO LOCAL ER AND DIAGNOSED WITH A FRACTURED RIB.
EE WAS COUPLING A ROCK DUSTER TO A 12 TON LOCOMOTIVE. AS HE WAS INSERTING PIN INTO PLACE, HIS CO-WORKER BACKED THE LOCOMOTIVE, MOVING PIN, FOWLING EE'S FINGER AGAINST THE LOCOMOTIVE FRAME (LEFT INDEX FINGER) LACERATING THE FINGER TO THE POINT OF THE AMPUTATION OF THE TIP FLESHY END OF THE FINGER.
EMPLOYEE WAS HAULING SUPPLIES WITH A RAIL LOCOMOTIVE TO THE #2 SECTION. CAR DERAILED WHILE HE WAS PUTTING THE SUPPLY CAR BACK ON THE TRACK. THE LIFTING JACK FAILED, CAR FELL, STRIKING HIM ON THE LOWER LEFT ARM.
EE WAS HAULING A LOAD OF ROCK TOWARD THE DUMP AREA. A LARGE BOULDER WAS POSITIONED TOWARD THE BACK OF THE TRUCK. IT CAUSED THE TRUCK TO UP-END. THE BOULDER ROLLED OUT OF THE BED AND SLAMMED BA CK DOWN TO THE GROUND WHICH SLAMMED THE DRIVER AGAINST THE STEERING WHEEL.
DRIVER WENT TO RT OF ROAD TO MISS HOLE IN ROAD & RT FRONT WHEEL SLID IN DITCH & TRUCK TURNED OVER IN DITCH.
#6 BELT SHUT OFF BECAUSE OF GOB, THUS EE WENT TO CHECK & FOUND CURTAIN BOARD AGAINST THE GOB SWITCH. HE TURNED #5 BELT OFF AT THE REMOTE SWITCH & WENT INTO THE ROCK BOX TO REMOVE THE BOARD. EE STATED THE BELT STARTED & CARRIED HIM ABOUT 7 BREAKS AT WHICH TIME HE JUMPED OFF.
EE WAS RUNNING A 488 SCOOP IN THE OUTBY AREA AT #6 BELT HEAD. THE SCOOP RAN OVER AN OLD TRACK TIE, CAUSING IT TO POP UP & HIT HIM ON THE RIGHT ARM CAUSING A LACERATION WHICH REQUIRED 9 STITCHE S.
EE WAS USING A 488 SCOOP TO UNLOAD A SCOOP BUCKET OF OIL CANS & GARBAGE, WHEN HE BACKED AROUND THE BLOCK, HE LOST CONTROL ON A GRAD, BUMPING HIS HEAD ON THE CANOPY.
EE WAS RIDING MANTRIP TO SECTIONS SITTING ON HIS KNEE WHEN MANTRIP SLID INTO MOTOR PARKED AT BRK 264 BELT CAUSING HIM TO GO FRONTWARD AND KNEE GO BACK UNDER HIM. NOTE: WHEN WE CAME BACK FROM V ACATION ON 1/2/02 WE WERE NOTIFIED OF THIS ACCIDENT.
EMPLOYEE WAS ABOARD THE MAN TRIP ON HIS WAY OUTSIDE THE MINE. PRESENTLY THE MANTRIP WAS STATIONARY. A 15 TON MOTOR VEHICLE CAME BACKWARDS ABOUT THE DISTANCE OF ONE BREAK AND BUMPED THE MANTRIP . THIS OCCURRENCE CAUSED CONTUSIONS AND MUSCLE STRAIN IN HIS LOWER RIGHT HIP.
EE WAS STANDING-LOADING A ROOF BOLTER WHEN A SCOOP BEGAN TO ROLL. IT PINNED HIS LEFT LEG AGAINST A BUNDLE OF ROOF BOLTS.
EE WAS A PASSENGER IN A BROOKVILLE MANTRIP PERSONNEL CARRIER AS THE MANTRIP PROCEEDED DOWN THE SLOPE, TOWARD THE TRACK STOP. THE OPERATOR LOST CONTROL OF THE VEHICLE. IT GAINED SPEED AND STOPP ED SUDDENLY AT THE TRACK STOP, PUSHING EE AGAINST THE INSIDE OF THE VEHICLE, CAUSING EXTREME PAIN IN THE BACK.
EE WAS DRIVING A LOADED COAL TRUCK ON HAULAGE ROAD DOWN HILL FROM STOCKPILE. THE THROUGH SHAFT TO THE REAR END BROKE. TRUCK GAINED SPEED, EE WAS UNABLE TO MAINTAIN CONTROL OF TRUCK. TRUCK WENT THROUGH A BERM, OVER AN EMBANKMENT AND OVERTURNED ON IT'S SIDE.
