EMPLOYEE WAS A PASSENGER IN THE OUT BY END OF A MANCAR, A PARKING MECHANISM FELL FROM ITS HOLDER TO THE MINE FLOOR CAUSING THE OUTBY END OF CAR TO RAISE ABRUPTLY WHILE DERAILING THE FRONT END OF THE MANCAR. EE'S MOMENTUM AND ITS ABRUPT RAISING AND LOWERING OF THE MANCAR RESULTED IN THE EMPLOYEE'S FALLING ONTO THE EDGE OF THE BACK REST STRIKING THE CENTER OF HIS BACK.
EMPLOYEE HAD STOPPED HIS DIESEL MANBUS (TRACK MOUNTED) DUE TO A BROKEN WATER LINE THAT HAD FLOODED THE TRACK, ANOTHER VEHICLE FOLLOWING, STRUCK THE PARKED MANBUS.
SHUTTLE CAR CANOPY HIT CABLE CROSSOVER BOARD & BOTTOM BOARD CAME AROUND & CAUGHT HAND AGAINST HAND HOLD
5B TAILPIECE AREA EE WAS STANDING ON THE 5B BELT ASSISTING ANOTHER EE TO PUT OIL IN THE 5A BELT SPEED REDUCER. THE 5B BELT STARTED UP WHICH CAUSED HIM TO FALL FLAT ONE THE BELT. THEN BELT TRAV ELED A 4 BREAKS DISTANCE WHEN IT WAS SHUT OFF WHICH ENABLED HIM TO GET OFF OF THE BELT.
SHUTTLE CAR RAN INTO SUPPLY TRAILER AND HE FELL BACKWARDS INTO A DIESEL SCOOP.
BRK, 52-3 WEST A BLOW BOY CARRIER THE SCOOP BUCKET SLIP SIDEWAYS ON THE LOWBOY AND CAUGHT HIS FOOT AGAINST A 3" METAL AIRLINE.
3 EAST LONGWALL PULLOUT (HEAD SIZE)-EMPLOYEE WAS POSITIONED BEHIND THE LONGWALL RETRIEVED MULE MOVING A DEWATERING LINE OUT OF THE WAY. WHEN THEMULE STARTED UP THE CABLE TOOK HIS FEET OUT FROM UNDER HIM CAUSING HIM TO FALL ON HIS RIGHT SIDE.
3 EAST LW-EE WAS WATCHING SLACK IN MONORAIL SYSTEM BEING PULLED DOWN FROM 2 BREAKS OUTBY. A 2" RETURN PRESSURE HOSE FELL OUT OF THE MONORAIL SYSTEM AND CAUGHT A BELT STAMP (IDLE) WHICH FOUL ED AGAINST A 4" METAL PIPE. THE 4"PIPE BROKE AT A VICTRAULIC COUPLING ALLOWING IT TO HIT THE EE IN THE RIGHT LEG AND SHIN AREA
2 EAST MAINS 4 & 5 DEV. SIDETRACK - EE WAS DISMOUNTING MOTOR WHEN THE TRAILING MOTOR SLID INTO THE BACK END OF THE TRIP WHICH THREW HIM OFF BALANCE.
OPERATING PERMISSABLE RIDE, HIT ROCK AND STEERING WHEEL CAUGHT RIGHT INDEX FINGER.
LOADING A LONGWALL SHIELD ONTO A SHIELD DOLLY USING A SCOOP CAR. THE DUCK BILL OF THE SCOOP CAME OFF THE DOLLY SUDENTLY, CAUSING THE SCOOP TO BOUNCE. HE STRUCK HIS HEAD ON THE CANOPY.
SOUTH 48 & 2 WEST MAINS SWITCH--EE WAS A PASSENGER IN A MANBUS WHICH SLID INTO ANOTHER MANBUS AT THIS TRACK SWITCH.
A FRONT END LOADER WAS LOWERING A LONGWALL RAM JACK RECEIVING GLAND TO THE GROUND. THE GLAN DSLIPEPD OFF THE FORKS OF THE END LOADER AND STRUCK EMPLOYEE'S FOOT.
HIS GOLF CART RAN INTO THE REAR OF A MANTRIP.
EMPLOYEE WAS A PASSENGER ON A PORTAL BUS WHEN IT STRUCK A PARKED 11 TON JEFFREY MOTOR. THE SUDDEN STOP CAUSED THE EMPLOYEE TO COMPLAIN OF PAIN TO HIS NECK, BACK AND HIP, PLUS HIS RIGHT SHOULDE R AND LEFT KNEE. X-RAYS WERE NEGATIVE. EMPLOYEE WORKED ON THE 5TH OF SEPTEMBER, BUT BEGAN TO MISS ON 9-8-97.
EE WAS TRYING TO STEADY A P40 PUMP IN SCOOP BUCKET WHEN THE SCOOP BOUNCED AND THE PUMP STRUCK THE THE TOP AND INJURED EE'S RIGHT THUMB AND INDEXFINGER.
EE WAS PUSING A PIECE OF ROC OVER IN PAN LINE ATJACK #135 WHEN A PIECE OF ROCK STRUCK HIM ON HISLEFT INDEX FINGER.
WHILE OPERATING A COAL HAULER EE HIT A HOLE IN ROADWAY CAUSING HIM TO STRIKE HIS HEAD AGAINST TOP OF CANOPY.
GARDEN PREP PLANT. A PIECE OF ANGLE IRON WAS BEING LOADED ONTO ENDLOADER FORKS. THE FORKS CAUGHT ON ANOTHER PIECE OF METAL AND CAUSED THE ANGLE IRON TO JUMP TOWARD EMP WHEN THE FORK CAME LOOSE . THE PIECE OF ANGLE IRON STRUCK EMPS LEFT LITTLE FINGER IN MID AIR.
WHILE ROCKDUSTING GOT ANKLE CAUGHT BETWEEN SCOOP AND RIB
1 EAST TRACK. EE WAS RIDING IN ELECTRIC SCOOTER WHEN THE SAFETY DOG BRACKET ON A RAIL CAUSING THE SCOOTER TO STOP SUDDENLY. EE DID NOT START MISSING WORK UNTIL 7/29/96.
WHILE DRIVING A THREE WHEELED MOVE CREW MANTRIP, THE INJURED PERSON WAS BACKING THE MANTRIP UP. AS THE MANTRIP WAS BACKING UP A PIECE OF ROCK STRUCK THE FRONT WHEEL CAUSING THE STEERING WHEEL TO SPIN. THE KNOB ON THE STEERING WHEEL STRUCK THE INJURED PERSON'S LEFT HAND. A FRACTURE OF THE CARPAL BONE BEHIND THE RING FINGER OCCURRED.
EE WAS TURNING THE TROLLEY POLE ON A TRACK SCOOTER WHEN SHE FELT PAIN IN HER RIGHT SHOULDER.
WHILE MAKING CHECKS AT THE 4TH 54" BELT DRIVE THE DOLLY CAR WRECKED AND BROKE A 1 1/4" WATER LINE. WHEN EMPLOYEE TURNED TO RUN HE TWISTED HIS RIGHT KNEE.
EMPLOYEE WAS OPERATRING A SKID LOADER IN ORDER TO CLEAN THE BOTTOM FLOOR OF THE PLANT. WHILE OPERATING THE LOADER THE CAB SLIDE EXTENSION GUARD FELL AND HIT HIS RIGHT MIDDLE FINGER.
EE STATED HE HIT A HOLE IN THE ROADWAY AND HURT HIS BACK WHILE OPERATING A COAL HAULER.
EMPLOYEE WAS AT MOUTH OF 3 DEV. HE WAS STANDING IN WHEEL BARROW CLEANING GREASE OFF BEARING AT BELT DRIVE, WHEN A TRACK VEHICLE CAME BY AND BUMPED WHEEL BARROW CAUSING EMP TO FALL AND HIT THE MINE FLOOR.
EE WAS OPERATING DUCKBILL SCOOP. THE LONGWALL LEAD DRIVE HE WAS LIFTING SLIPPED OFF OF THE DUCK BILL WHICH CAUSED THE SCOOP TO BUCK. HE WAS THROWN UP AND HIT CANOPY.
EMPLOYEE WAS STANDING IN STAGELOADER CONVEYOR WHEN THE CHAIN WAS BUMPED FORWARD HIS FEET WERE KNOCKED FROM UNDER HIM & HIS RT FOOT WENT UNDER A 4FTX4FTX2FT+ THICK PIECE OF ROCKLEDGED IN THE ST AGELOADER CONVEYOR
A MOTOR HAD STOPPED FOOR SOME BELT EMPLOYEES ON MAIN, TRACKWAY. WHEN A MANBUS TOPPED OVER A SLIGHT GRADE THEY SEEN THE MOTOR. BUT COULD NOT STOP AND STRUCK BACK OF MOTOR. EE BUMPED HIS LEFT KNEE AGAINST MANBUS.
EMPLOYEE WAS OPERATING A COLA HAULER. WHEN TAKING A LOAD TO THE TAIL PIECE EMPLOYEE RAN THROUGH A HOLE IN ROADWAY AND JARRED HIS NECK. EMPLOYEE DID NOT START MISSING WORK UNTIL 7-31-95.
EE WAS OPERATING A DIRSAL LOCOMOTIVE ALONG THE 2 EAST MAIN TRACKWAY. WHILE TRAVELING PAST THE INTAKE AIR SHAFT AT BREAK 70' SOMETHING BLANK INTO HIS RIGHT EYE. HE WAS WEARING EYE PROTECTION. H E USED PRESCRIPTION MEDICATION AS TREATMENT FOR FOR THE INJURY.
EMPLOYEE WAS OPERATING A TROLLEY MOTOR (EMPTY WHEN IT DE-RAILED ON A TRACK TURN AND WENT UNDER AN OPERATIVE BELTLINE.
EMPLOYEE WAS SITTING IN A PARKED MANBUS WHEN ANOTHER PARKED BUS DRIFTED INTO THE RIDE - DRIFTED APPROXIMATELY 15-18 FEET.
INJURED WAS BACKING UP A GOLF CART AND BACKEDINTO A PARKED SHUTTLE CAR CAUSING A STRAIN TO LOWER BACK.
