Showing all 29Employee was following a Tractor Trailer on the haul road and was temporarily blinded by sun glare causing him to contact the tractor trailer in the rear end.
The injured was behind mechanic truck #651 when the 854 G push-cat struck the mechanic truck and pushed it into 789 C rock truck #597 injuring his right forearm that was caught between the mechanic truck bumper and the wheel of the rock truck.
The driver was beginning to dump in the top truckdump at the Consol No. 1 plant and while dumping the truck turned over.
The driver was hauling coal to the plant from spruce creek was starting down a hill the truck got out of gear and the engine quit on the truck he could not get it back into gear he went down the hill and turned over.
Employee stated while operating shuttle car he ran over a rock in the roadway bouncing him against canopy. Employee sustained compression fx to the L1 vertebrae.
EE was positioned between #3 mobile bridge and the low-low structure, right foot on the low-low belt, left foot on mine floor. EE was attempting to bump conveyor chain, in this process, his arm or shoulder engaged the right tram causing the bridge to move forward, catching his left leg between the mobile bridge and low-low structure. Fractured his fibula and cut his calf (sutures).
Employee was tramming scoop through block curtain battery end first when curtain came in contact with canopy. Employee said his vision was blocked. He raised curtain up with his left hand inching scoop thru curtain when his left hand got caught between rib and canopy. Employee sustained fractures to his index finger and thumb.
EE was swinging lare componet (777A Brake unit) with service truck boom. He was watching the top of the boom to check clearance while swinging boom. As a result he was trapped between compartment and service truck.
Ee sustained injury to his left big te when an airlock door came in the deck of personnel carrier he was riding in EE sustained crushing injury to his left big toe. EE big toe was amputated to the first joint.
Employee was loading supplies onto motor, operator told him he was moving. Injured employee did not realize how close he was to motor. The motor caught the tip of his left boot and rolled it over against the ground, causing an injury.
EE WAS HELPING INSTALL BELT IN #1 BELT LINE. A DIESEL MOTOR WAS BEING USED TO PULL THE LOOPED BELT OFF A FLAT CAR. THE BELT SLIPPED OFF THE FLAT CAR, STRUCK EE, SPUN EE AROUND AND INTO THE BELT STRUCTURE CAUSING INJURY. EE WAS POSITIONED BEHIND THE FLAT CAR AT THE TIME OF THE INCIDENT
Employee pinched fingertip while riding in scoop bucket, possibly by ejector, employee was not sure. The ejector was chained and had two inches of clearance. Employee was wearing metacarpal gloves.
EE was backing bridge system out of a cut, while doing so the outby edge of operator's canopy caught a roof bolt head, throwing EE against arm rest and panic bar, causing injury.
EMPLOYEE STATED THAT HE JARRED HIS BACK WHILE OPERATING A SCOOP. FELT PAIN IN LOWER BACK. DID NOT START MISSING WORK UNTIL 11/23/04.
EMPLOYEE RIDING IN SCOOP WITH CONCRETE BLOCKS. SCOOP RAN INTO MINER TAIL, MOVING BLOCKS OVER ONTO HIM.
EE WAS INJURED WHEN STRUCK BY AIR LOCK DOORS AS MANTRIP WAS PASSING THROUGH.
EMPLOYEE WAS SITTING ON MANTRIP WHEN ANOTHER MANTRIP COULD NOT STOP AND STRUCK THE MANTRIP SITTING CAUSING EMPLOYEE TO HURT BACK.
EE WAS RECOVERING A 4" WATER LINE AT TURTLE PORTAL USING A BATTERY POWERED SCOOP, HAD WATER LINE TIED TO SCOOP BUCKET WITH ONE JOINT OF PIPE EXTENSION BACK OVER SCOOP ON OFFSIDE WHEN PIPE GOT HUNG ON CANOPY POST AT PORTAL CAUSING PIPE TO SWING AROUND STRIKING EE IN THE FACE. EE SUSTAINED A FRACTURED NOSE & LACERATION ABOVE HIS RIGHT EYE.
OPERATOR DECK OF SHUTTLE CAR CAUGHT THE SIDE OF A PIECE OF DRAW ROCK WHICH WAS IN AN INTERSECTION ANS SHOVED IT AROUND ONTO MINER HELPERS LEG CAUSING INJURY. THE DRAW ROCK HAD BEEN CLEANED OFF THE TOP OF THE CONTINUOUS MINER.
EE STATED WHILE OPERATING MOBILE BRIDGE HIS LEFT KNEE STARTED HURTING. EE SUSTAINED A SPRAIN/STRAIN TO HIS LEFT KNEE.
EE WAS SUPERVISING & ASSISTING IN PULLING BROKEN BELT BACK TOGETHER. BELT WAS BEING PULLED BY A SCOOP W/ROPE TIED TO BELT, A CHAIN HOOKED TO ROPE. DURING PROCESS OF PULLING, THE BELT GOT HUNG CAUSING CHAIN TO BREAK. THE HOOK & 8" OF CHAIN TRAVELED ABOUT 14 FEET STRIKING EE ON HIS LEFT ANKLE. EE SUSTAINED A FRACTURE TO HIS LEFT ANKLE.
EE WAS RIDING IN MANBUS TRAVELING TO "A" SECTION. WHEN EE ARRIRVED AT A-4 HEAD DRIVE ANOTHE MANBUS WAS PARKED AT HEAD DRIVE. EE GETS OUT OF MANBUS HE WAS RIDING, WALKS TO THE PARKED MANBUS. TH E MANBUS HE HAD BEEN RIDING IN DRIFTED TOWARD PARKED MANBUS, CATCHING EE BETWEEN THE TWO MANBUSES. EE SUSTAINED FRACTURE TO HIS LOWER RIGHT LEG.
EMPLOYEE STATED THAT FLATCAR ROLLED BACK CATCHING LEFT LEG BELOW KNEE REQUIRING 3 SUTURES.
EMPLOYEE STATED WHILE PREPARING TO CHANGE OUT BEARING ON B2 BELT TAIL ROLLER A PIECE OF ROCK FELL FROM THE MINE ROOF STRIKING HIM ON TOP OF HIS HEAD KNOCKING HIM TO THE MINE FLOOR. EMPLOYEE SU STAINED COMPRESSION FRACTURES TO THE T7&T12 VERTEBRAS
EE STATED WHILE WORKING AT THE #5 HEAD DRIVE INSTALLING FIRE DELUGE SYSTEM WAS ON OFF SIDE OF BELT, HAD CONNECTED SYSTEM UP TO THE OFF SIDE WAS INE THE PROCESS OF PUTTING DELUGE HOSE ACROSS BE LT BETWEEN THE TOP AND BOTTOM BELT WHEN HOSE GOTCAUGHT JERKING HOSE AND EMPLOYEE'S RIGHT HAND INTO DISCHARGE ROLLER. EE SUSTAINED FRACTURES AND LACERATIONS TO RIGHT HAND.
EE WAS ATTEMPTING RO RELOCATE ROOF BOLTER CABLE DUE TO CANOPY OF MBC GETTING CLOSE TO CABLE AS MBC WAS PASSING UNDERNEATH CABLE. EE HAD STOPPED HIS BRIDGE REACHED OUT WITH HIS LEFT HAND TO REL OCATE BOLTER CABLE DURING THIS PROCESS THE INBY BRIDGE BUMPED EE 'S BRIDGE CAUSING CANOPY TO BE JAMMED AGAINST MINE ROOF. EE'S LEFT HAND WAS CAUGHT BETWEEN CANOPY & ROOF. EE SUSTAINED CRUSHIN
EE WAS ATTEMPTING TO DISMOUNT PERSONELL CARRIER WHEN PERSONELL CARRIER MOVED CATCHING EE'S LEFT LEG BETWEEN PERSONALL CARRIER & BELT STRUCTURE LAYING ALONG SIDE OF TRACK. EE SUSTAINED A FRACTURE TO HIS LOWER LEFT LEG
EMPLOYEE EXITED TRUCK POSITIONING HIMSELF ON THE GROUND NEAR A PENDANT MOUNTED CONTROL STATION FOR AN ELECTRICALLY OPERATED GATE WHICH PERMITS LOADING OF COAL COARSE REFUSE FROM A 175 TON STORAGE BIN. AS EMPLOYEE OBSERVED LOADING OPERATION, A SMALL PARTICLE OF ROCK BOUNCED OFF THE TRUCK STRIKING HIM IN THE RIGHT EYE CAUSING AN ABRASION. WAS NOT WEARING SAFETY GLASSES.
