Showing 50 of 94A mantrip containing 8 employees was traveling outby on the 9W roadway. While turning onto the 8W roadway the driver turned the mantrip too wide, struck the outby corner of the intersection, resulting in one employee contacting the metal frame resulting in a fractured tibia.
IE stated EE was backing a tractor with an attached trailer down hill. EE stated EE put EE's hand on the frame on the frame of the machine, outside of the cab, to help turn EE's body toward the direction of the trailer. EE accidentally depressed the throttle causing the tractor to turn slightly, smashing EE's finger that was resting on the machine, causing a fracture.
While tasked to fuel pit equipment, the employee crested ramp going to 5 pit when EE lost the front brakes. This caused the truck to descend ramp faster than normal with just the rear brakes working. The employee experience neck pain later in the shift.
IE stated that while EE was operating a man trip, EE reached over to turn the lights off due to oncoming traffic. At that time, the IE ran over a large rock causing the IE to be lifted off EE's seat, striking EE's head on the top of man trip, then back in EE's seat. The IE stated EE did not feel immediate pain. On 4/19/2021 IE was taken off work for further evaluation.
An employee was pinned against a coal rib by a coal hauler. The IE had walked to the #5 entry to assist another operator establish an alignment marker on a coal rib. As IE was walking back to IE's equipment, hauler (B) was traveling from the #4 entry towards the feeder and contacted the parked hauler (A) located in #5 entry, allowing hauler (A) to slide, pinning the IE to the rib.
While driving a ram car on unit 1 the employee bumped their head against the canopy of a ram car. The IE stated that they drove through a hole which caused them to suddenly come up and in contact with the canopy.
IE was running a continuous miner. IE was cutting extra height for a future feeder setup. IE noticed blood on IE's pant leg and realized IE had been cut.
IE was positioned inside the trailer end of the coal hauler trying to find a blown hydraulic hose. The operator was prompted to bump stinger to locate the blown hose. When stinger was engaged the auto reset button was pushed instead of the manual, causing the stinger to retract back towards the IE pinning the IE between the stinger and the back of the false bottom.
IE tramming coal hauler towards continuous miner to get cleanup car in entry # 7. As they were tramming towards continuous miner, they ran over a metal roof strap in intersection, just outby where continuous miner was cutting. Strap came into operator's compartment striking IE on side of the rib cage below arm pit causing a laceration.
While driving the Getman tractor along the travelway, the IE stated IE ran over a rock that was located in the wind row left behind the grader. The IE stated it caused IE to bounce in IE's seat. IE stated IE felt a sharp pain in IE's lower back extending down to IE's right leg. IE continued to work until IE had surgery on Dec. 27, 2018.
IE was driving a mantrip on the 4th SE travelway when they fell asleep . A passenger yelled and they looked up as the mantrip hit the rib, broke the 4" discharge waterline on the opposite side of travelway. The discharge waterline came through the windshield striking IE causing a laceration of the left triceps.
A Getman tractor operator pulling an empty trailer ran over undulations in the floor and struck head on a newly installed canopy. EE thought they were fine and continued but said the pain worsened and neck stiffened so they notified the supervisor. Employee was taken for medical examination and released to work full duty but on 5/9/2017 began working under restrictions.
A temp employee was hauling gob with a diesel scoop. EE reported to EE's foreman something hit EE in the back of the head while driving the scoop back to a turn around. The foreman noticed signs of concussion and initiated response. The area involved has been checked twice by an investigation team. It is believed the TE struck EE's head on the top of the seat or on the mine roof.
IE was backing in a unit fuel tank in a xc with a tractor when ee pinched right pinky in a pinch point where the tractor sections come together. EE backed over a berm causing the rear section to lift up butting against the front section causing a laceration.
End Dump back through berm and onto spoil bank, when truck stopped driver slid out from seatbelt and struck head on cab frame.
IE was waiting between entry 7 & 6 for a coal hauler to tram towards the feeder in entry 5. When the coal hauler started tramming towards the feeder it caught a roof bolter cable, started pulling it down behind the IE, striking the IE in the back. IE was transported to medical facility. IE was re-evaluated the next day and was unable to return to work on EE's next scheduled shift.
Mantrip 100004 did not make a turn exiting the mine and struck the rib. A passenger said they experienced back pain. The employee was examined at the local ER and taken off work.
Employee said while examining on a permissible ride in the 3A panel, they ran over a rock in the path of travel. When this occurred the steering wheel turned unexpectedly. The employee stated they did not let go of the steering wheel and began having shoulder pain. It was determine the employee had a torn rotator cuff and had surgery on 9/20/2016.
IE was pushing a disabled mantrip from the side of the machine into a crosscut with another mantrip, when the disabled mantrip bumper slid down the side of the mantrip the IE was operating and struck him in the knee.
IE was operating a ram car when he ran over a piece of coal. Apparently part of it broke off and came into the cab and struck him on the right side of his nose. It took 6 sutures to close the laceration.
A wheel unit and tire broke off of a coal hauler, causing the operator to be bounced into the canopy of the hauler injuring his back.
A repairman was driving a golf cart to retrieve his tools at the start of shift and caught his left foot on some curtain that was lying on the bottoms causing his leg to twist fracturing his tibia.
IE was riding in a mantrip traveling to the bottom at the end of his shift. An unknown object struck him in the right shin. He received a laceration to his right shin requiring 8 sutures to close the wound.
IE stated he was tramming coal hauler from the face back to the feeder and he ran over a crib tie and bounced up into the canopy injuring his neck. Due to several doctors and tests not finding anything wrong, we have waited until a IME was performed. The doctor from the IME verified his problems could have come from the accident.
I.E. was checking master switch on #12 Getman tractor when it suddenly rolled forward pushing his left leg under a mantrip that was parked in front of it. The injured sustained a fractured left fibula. The incident occurred on the 3 M.E. at XC #47.
Walking up to mantrip to hook up tow rope. Not knowing the ram car was in reverse coming toward him. He was pinned in between the ram car and mantrip.
IE was backing a tank duster on the travelway using a Getman Tractor, as he attempted to set the duster over the tractor articulated and caught his right elbow between the cab and back section of the tractor. The injured suffered a fracture to his humerus (funny bone) that will require surgery to repair.
The injured employee was traveling up the South Slope operating a Getman Tractor. The tractor suddenly lost all power and the brakes failed to work. The employee and tractor traveled backwards down the slope and struck a coal rib damaging the tractor. The employee suffered a contusion to his left shoulder and missed 4 days of work.
The injured was operating #96 wallace mantrip, the vehicle hit a bump in the road way causing the injured to strike his head on the canopy. The injured was treated for a neck strain and released without restrictions. On 7/13/2012 he was taken off work due to headaches and diagnosed with occipital neuritis.
IE attempted to couple an empty oil trailer to a tractor on Unit #1. As he picked up the trailer tongue, the trailer moved downgrade. His right leg was caught between the oil trailer and the tractor. A chock became disloged from the empty trailer when a loaded trailer was spotted beside it. He sustained a compound fracture to the tibia. An EMT on Unit #1 administered first aid.
Injured employee was pushing a dead coal hauler out of the way with his ramcar and the hitch on his went into the bucket of the one he was pushing. This car went over a small hill in travelway and raised his off the ground. When it came loose and his coal hauler hit the ground it threw him up into the canopy of his ramcar, causing multiple injuries.
The injured was building a stoppage utilizing 6" solid blocks between the #5 & #6 entries. A coal haulage operator with a load drove through the curtain & struck the construction. The resultant collision knocked the blocks onto the injured. He suffered a severely lacerated right calf, a broken fibula & a broken patella. The injured was air & land evacuated to the U of L hospital.
Employee had left his scoop bucket out in travel way. A coal hauler came along and struck his bucket and knocked it around into the rib. He was between it and the rib. Broke bones to sacrum and tail bone areas.
The IE stated that he was changing the batteries out on the coal hauler he was operating. When he was tramming away from the set of batteries he ran over the jumper cable with his front tire causing the cable to get tight, pinching his hand between the edge of the fender and the cable.
The employee stated that he was traveling around a corner in # 7 entry in a battery coal hauler, when his front wheel climbed on top of an uneven ledge. This caused the coal hauler to slam down jarring him in his seat. He stated that he was having lower back pain and right leg pain.
The injured was operating #85 Damascus, battery operated 2 man vehicle. He and a passenger were traveling down the South Slope. The vehicle struck the corner of the #9 shelter hole causing the injured to strike the steering wheel suffering injuries to his chest. He was transported by ambulance to Good Samaritans hospital, and later transferred to Deaconess.
The injured was a passenger on a Damascus MAC-XP ride that struck the #9 shelter hole in the South Slope. He was transported to Good Samaritans Hospital and later transferred to Deaconess in Evansville. He was treated for a laceration to his upper lip and and an avulsion to his left leg and released.
As the CAT 777 end-dump driver was travelling from the pit floor to the top of the coal seam (approx. 8 foot incline), he was jarred inside the cab of the truck resulting in a compression injury to his lower back.
Employee was operating ramcar and ran over rock in roadway and hit his head on canopy of ramcar, straining his lower back.
Employee shut hs rt.ring finger in the door of a Cat 777 which resulted in a avulsion to the tip of his right ring finger,which required sutures to repair.
The injured, was cleaning belt structure on the 1 "A" belt at crosscut #1. He was using his gloved hands wiping dust off when his glove got caught between the troughing roller and belt. The incident caused a partial amputation of the ring finger of his right hand just above the first joint.
Employee was riding out of the slope in a diesel mantrip when the trip started to slid backwards down the slope. After several attempts to stop the trip, the driver turned towards the arches to stop the ride. The impact caused EE, who was sitting in the last seat behind the driver to have contact the back wall of the mantrip.
Injured man was on his way to work for the 9-19-08 midnight shift at the Francisco underground mine. After turning off a county road onto the mine access road he failed to yield to a large coal hauler truck coming from the left. The large truck struck his vehicle causing several injuries.
The injured was operating a scoop and was in the process of pulling into a crosscut to get some boards to build cribs with. As he drove the scoop into the crosscut, he ran over some loose roof bolts and one of the bolts came up into the operators compartment and struck him in the mouth. The incident cut his lip and broke off four teeth.
On 7/16/08 the injured was a passenger in a 14 man diesel mantrip. The mantrip he was traveling in attempted to go around a parked scoop when the passenger side bumper came in contact with the scoop. The injured bumped his left shin on the metal partition between seats. On 7/31/08 the injured was diagnosed with cellulitis to the injured area.
The accident occurred in the slope approx. 1260' from the surface when the examiner was heading down the slope in a Wallace 6 man diesel mantrip. The injured lost control of the vehicle, and struck on of the supporting arches with the front drivers side of the mantrip. The injured walked to the surface and called for help.
While cleaning ice from frame of belt line at bin roller, employee's hammer became entangled between belt and roller pulling hammer and right hand of employee in behind it. Employee sustain an avulsion to the right middle finger that required surgery to repair.
Injured was driving a golf cart on #1 unit. He hit a rock in the roadway which caused the steering wheel to jerk and strain his arm. He continued to work until 1/16/2008 when he received an operation for a torn tendon in his left arm.
After fueling EE entered the cab. He reached for the seatbelt between the seat and buddy seat, which is over the heater/air conditioning unit. The guard for the fan was missing, allowing his finger to come in contact with the fan blades. 6 stitches were required to close the incisions.
