THE EMPLOYEE WAS RIDING ON A RUBBER TIRED PERSONNEL TRANSPORTER HE SAT UP TO TIE HIS BOOTLACE AND STRUCK HIS HEAD ON THE HEAD OF A CABLE BOLT. THIS CAUSED HIS HARD HAT TO STRIKE HIM ON THE BRIDGE OF HIS NOSE RESULTING IN A FRACTURE.

EE WAS TRAMMING A RUBBER-TIRED UTILITY VEHICLE AND DROVE OVER A 4'-LONG METAL ROOF JACK. THE METAL ROOF JACK FLIPPED UP AND STRUCK THE EE ON THE HEAD, RESULTING IN A MILD CONCUSSION. An investigation revealed that the alleged injury was falsified by the employee.

THE INJURED PARKED #1 S/C IN THE #3 INTERSECTION TO HELP MOVE THE MINER CABLE OUT OF THE HAULAGE ROAD; AS HE WAS RETURNING TO HIS S/C, THE #2 S/C WAS APPROACHING HIM FROM THE X-CUT (BETWEEN #1 -3 ENTRIES). OPERATOR OF #2 CAR DIDN'T SEE THE VICTIM & RAN INTO THE #1 CAR, CATCHING THE VICTIM'S LOWER LEGS BETWEEN THE TWO SHUTTLE CARS.
