Employee injured servicing GM Autocar fuel truck when an unplanned movement of the truck occured. Employee was struck by the rear bumper of the fuel truck while walking between the rear of the truck and partially closed shop door. Employee sustained a broken pelvis

Empty haul truck was heading back to cut, when met by oncoming truck, which was taking up too much of the road. Moved over too far and got into muddy ditch. This made him slide sideways, jackknifing truck on left hand side of road slamming him around inside cab. Caused broken arm, ribs and head laceration.

While hauling topsoil to dump area employee noticed something inside his air conditioner (a piece of paper). He took the louver off of the A/C unit & reached inside the unit & put his finger into the fan, stopping the fan & shorting out unit. Caused laceration to right ring finger requiring medical assistance.

EE was attempting to dump #2044 haul truck into circuit #1 and #2 coal hopper. The bed went up approx 6 ft and stopped. He attempted to feather the bed down with the controls. The bed slammed onto the frame and he injured his lower back. He returned to work full duty for three shifts, but the pain continued. He requested the Dr and missed his next regular shift 3-9-05.

Ee was riding on a mine pit bus when something entered his right eye. He was taken to Campbell County Hospital & diagnosed with a granular substance in his eye. It was swabbed & he was given a prescription antibiotic to prevent infection. He then returned to work that night with no restrictions.

EMPLOYEE WAS IN THE LEAD SCRAPER PUSHING THROUGH AN OVERBURDEN REGRADE AREA. THEY HAD FINISHED LOADING AND WERE GOING DIRECTLY TO THE FILL AREA TO BEGIN DUMPING THEIR LOADS. EMPLOYEE WAS BUMPED BY THE REAR SCRAPER AS THEY WERE COMING UNHOOKED. HE WAS LOOKING BACK TO ASSESS THE SITUATION AND DID NOT RECOGNIZE A ROCK IN HIS PATH. HE DROVE OVER THE ROCK WHICH IMPACTED HIS BACK.

EE was training a truck driver who lifted the bed of the haul truck approximately 2-3 feet short of the hopper. EE tried to feather the bed down fully loaded. The bed slammed down, causing pain in his neck. He continued to work restriction free until we received a doctors restriction on 9/13/04 to not drive haul truck due to a disc lesion diagnosed in an MRI.

PIT BUS #2281 WAS DRIVING EMPLOYEES TO THEIR RESPECTIVE VEHICLES PARKED IN THE PIT. THE BUS DRIVER HIT A DIP IN THE ROAD & THE EE IN THE BACK SEAT SUSTAINED A SOFT TISSUE LOWER BACK INJURY.

THE EE WAS OPERATING A SCRAPER WHEN HE DROVE UP AND OVER A LARGE ROCK. THIS JARRED THE EE AND CAUSED A BACK STRAIN.

THE DOOR TRIPPED OPEN ON THE LOADED BUCKET OF AN OVERBURDEN SHOVEL AS IT SWUNG OVER THE BACK END OF A HAUL TRUCK. THIS CAUSED THE TRUCK BOX TO TIP UP AND THEN DROP, JARRING THE TRUCK DRIVE AND STRAINING HIS NECK AND BACK MUSCLES.
