Employee was sitting in passenger compartment of CO #51 mantrip which was located at the mouth of the hole waiting for clearance. While the mantrip was sitting still, it was struck from behind by CO #84 mantrip. Employee stated that this jarred him causing pain to his lower left back. The seat the employee was sitting in had no back to it.

Injured employee was operating a shuttle car from the #4 entry to the feeder in the #2 entry. While tramming off a bridge placed on the mine bottom, the shuttle car struck the mine bottom causing a jarring of the employees lower back.
Injured employee was struck on the knee by a shuttle car cable. The shuttle car was stuck in the mud on the section and the scoop was being used to attempt to pull it out. The employee was standing behind the shuttle car and as the pump motor was activated the slack was pulled up and the cable struck his knee. Employee did not begin missing work until 04/21/2014.
Employee was operating a shuttle car and while attempting to turn into a left crosscut he struck his left arm on the rib causing a sprain/strain type injury to his left shoulder.

Employee was operating #67 bus. The control switch was going in and out. The tram power would not stay on. Employees were pulling the pole off the wire to slow the vehicle down. This was done many times once causing injured employee to be thrown against the pipe guard around the windshield of the vehicle causing him to strike his head.
IE was operating scoop attempting to remove sections of pan line from the 16 left longwall face area. The section of pan line he was attempting to pull using a winch on the scoop had not been disconnected from the adjoining section of pan line. When connection broke the scoop jarred causing him to strike his head against the scoop canopy.
EE stated that while releasing the brakes on a mantrip it was struck from behind by another mantrip causing him to fall forward. The IE continued to work with intensifying pain in his lower back as the shift progressed.
Company # 808 scoop was being trammed though a curtain which had been rolled up on 1 South Mains. The curtain became caught on one of the headlights of the scoop. A fellow EE raised the curtain off the scoop. The scoop operator proceeded to tram the scoop and the injured EE was struck on his right foot by the tire of the scoop.
EE was being transported to the mine bottom on company #73 bus due to an illness. During transportation the bus came off track at 1 South #1 spur switch causing the EE to strike his head on a metal cover located behind the operator and strain his neck.
Employee was in company #58 portal bus while attempting to push company #53 bus was off track through a switch. While doing so the injured stated he struck his lower back on the window of the portal bus, causing a strain to his back.
EE was sitting in 54 portal bus at the "hole" switch. 58 bus moved from the spur to the mouth and didn't see 54 bus sitting there. 58 bus struck 54 bus causing EE to receive whiplash.
Employee stated he was tramming S/C #221 through section scoop doors. While doing so he had his right elbow outside the confines of the canopy. This caused his elbow to be caught between the scoop door and frame of S/C.
The #11 motor was pulling the track duster to the James Creek Section while being followed by the #10 motor. The #11 motor "saddle bagged" in a dip and was struck by the #11 motor. The employee that was riding as a passenger in the #10 motor was diagnosed with a lumbar strain and contusion to the left knee. Medical diagnosis was not received at the mine site until 8-20-2009.
The employee stated he was tramming scoop and hit a hole in the roadway which jarred his back. He took the scoop inby one crosscut and got hung up. Crib blocks were placed under the wheels and when he trammed off the blocks it jarred his back.
The employee stated he was operating a shuttle car and ran over a rock. He got out of the shuttle car to move the rock and when he bent over he felt a pain in his back. He stated he did not touch the rock. He had been diagnosed with degerative changes in his back.
The employee stated he was in a mantrip coming outside. The operator ran into the back of another mantrip that had stopped in front of their trip to let a motor come out of the side track. The employee has been diagnosed with degenerative disease in the back.
The employee stated that he was sitting in the deck of a locomotive when the safety chain hook caught his finger between the frame of the locomotive and the hook and avulsed his little finger.
Employee stated he was tramming a shuttle car in the #2 heading across bridges installed to maintain the roadway, While doing so he stated the canopy of the shuttle car struck the top causing a piece of rock or metal to strike his right eye.
Supervisor and witnesses were attempting to remove the gearbox from the 3 Right Longwall face. While doing so the tensioner motor was knocked loose by the scoop bucket and struck the injured employee on the lower left leg fracturing it in 4 places.
The employee stated he was operating a shuttle car and had parked in a cross cut to switch out with the other shuttle car. He started to pull into the entry when he felt pain in his left arm. He was not aware of what happened but did sustain a scratch and bruise on his forearm. He has been diagnosed with arthritis of the elbow.

EE stated he was running #5 Coal Hauler to feeder with a load of coal, when he made his turn to the feeder and changed direction to get on the feeder. The center section of the coal hauler broke apart, causing him to strike his head on the canopy and injuring his neck.
The employee stated he was aligning endloader to pick up loose bolts out of a bolt box when the fork hung on the frame of the box and caused the box to bounce and it struck the employee on top of his foot.
The employee stated that he was operating the scoop cleaning the roadways when the scoop hit a rib board and caused the board to strike his head.
The employee was operating a coal hauler when it hit a hole in the bottom causing him to strike his head against the canopy. Due to unavailability of the doctor and the mine being on miner's vacation, this incident is now being reported. The employee has degenerative changes along his spine.
The employee stated he was asked to pick up some supplies on the Mac-8 mantrip. He was driving in front of the bathhouse when he tried to avoid the speed bump in the road and struck the guard rail. He stopped suddenly and contacted the steering wheel with his chest. He was diagnosed with four fractured ribs.

The employee stated he was on the longwall face traveling from the tail towards the head. The shearer was parked and they were running the face conveyor. He had stopped at the shearer tail drum to push a rock off the panline when another rock in the conveyor struck the tail side spray bar and caused it to strike him on the side of the face.
The employee stated he was sitting in a mine jeep when a locomotive struck another vehicle which struck his in a chain reaction. The employee was diagnosed with degenerative changes to his left knee and left elbow.
Employee was operating #40 shuttle car tramming towards the feeder. Approx. 100' from the feeder the tram pedal stuck & he ran over a rock 3'x 2'x 4". This force caused the employee to strike his head against the canopy of the shuttle car. EE did not begin to miss work until 8/11/06
EE was operating shuttle car when he felt a pain in his back. EE has history of trauma. EE has a history of low back pain. He has spinal differences at S1,L1,L2,L3,L4,L5,L3,L4. EE had MRI on 3/25/05. where these results we identified.
The employee stated he was loading a piece of curtain in the scoop bucket when the scoop operator dropped the scoop bucket and it landed on his left foot that was placed approximately 6" under the edge of the bucket.
The employee stated he was operating a portal bus when the pole base contacted a rib board and caused the pole to swing around and strike him on the shoulder.
AS EMPLOYEE WAS DRIVING TRUCK DOWN TO COW CREEK ROAD (APPROX 15 MPH) TO CHECK THE PONDS, HE ATTEMPTED TO NEGOTIATE A TURN TO THE RIGHT. THE TRUCK WHEELS DID NOT TURN AND HE RAN UP ON A CONCRETE BARRIER. THIS RESULTED IN A CONTUSION TO THE LARGE KNUCKLE AT THE BASE OF THE INDEX FINGER ON THE RIGHT HAND.
EE STATED WAS OPERATING A BOBCAT LOADER BESIDE THE RAW COAL SILOS WHEN EE WENT OVBER AN EMBACKMENT INTO A STREAM
EE was operating the portal bus to switch out of the way for the supply motor. The motorman lost control of the motor and 3 cars. He hit the bus causing him to strike his right eye on the sander bar. He received 9 sutures.
The employee was observing a locomotive pull a wrecked move car with the longwall tail drive on it to the track using a chain hooked into the pull eye when the pull eye broke off the car and glanced off the locomotive and struck the employee on the forehead.
The employee was operating No. 9 locomotive and was cleared up in the side track when his pole came off the wire. When he grabbed the pole, it broke and pushed his hand against the mine roof.
THE EMPLOYEE STATED HE WAS WATCHING A SCOOP PULL THE BRETBY CHAIN IN THE PAN LINE USING A 3/4" NYLON ROPE WHEN THE ROPE BROKE AND STRUCK HIM ON THE LEGS.
EMPLOYEE WAS OPERATING TRUCK ON A HAUL ROAD AT FEDERAL #2, 6 LEFT SHAFT SITE. HE WAS TRANSPORTING A LOAD OF GOB FROM THE SHAFT TO CUTTINGS DUMP SITE. HE LOST CONTROL OF THE VEHICLE AND RAN OVER A SUBSTANTIAL BERM AND LANDED BELOW THE HAUL ROAD.
