Showing all 7Employee was moving waterline with scoop, caught waterline with scoop, causing waterline to come up on top of the scoop. Water line hit the top of his head on the left side , sutures were required.
Employee was helping guide the low track fork lift, the operator was trying to pick up a load of resin. When he attempted to back up the low track slid and the fork struck EE's left leg in the knee area. He is on restricted duty. X rays and cat scan were taken no broken bones. He returned to work on 12/15/2005.
Employee was operating a diesel buggy on the track. The buggy hit a broken rail and it threw her forward and her head struck a piece of metal. She got cut below her left eye and had seven sutures. She returned to work the next scheduled shift.
EE WAS OPERATING A BUGGY ON THE TRACK AND STRUCK ANOTHER BUGGY PARKED ON THE TRACK. THE LID OVER THE MOTOR FLEW UP AND CAME DOWN ON HIS MIDDLE FINGER ON HIS LEFT HAND. HE HAD A LACERATION AND BROKEN BONE IN THE END OF HIS FINGER. HE WAS TREATED AND RELEASED TO WORK THE NEXT SCHEDULED SHIFT. HE WAS TREATED ON JAN 31ST.
RAN OVER A BLOCK OF COAL OR ROCK THAT HAD FALL IN ROADWAY THAT HE DIDN'T SEE; CAUSING LOWER BACK PAIN.
WAS DUMPING COAL AND TRACTOR & TRAILER TURNED OVER AND BUMPED HEAD OF EE.
