Accident type, without injuries

While pillaring in #1 entry, 15th row at the left wall, a roof fall has occurred on the Continous Joy Miner. The size of the fall was approx. 19'wide x 8'thick, x 35'long. No ventilation has been effected or miners hurt. Rope bolts, beams, and jacks have been set and additional roof support of 5'and 10' rope bolts will be in installed.

While mining #4 pillar at brk. 24 a roof fall occured on the continous Joy miner, the size of the fall was approx. 11.5'W x 10H x 8' thick. 48"" resin bolts and 7' timbers set ,tests holes & straps according to plan. No ventilation affected or miners hurt.

A rock fall was found approx. 12:40 pm in the #1 main return, in #1 entry between brk. 48-49 in the secondary escapeway, the fall was approx. 100' long & 8' thick, the area has been dangered & timbered off, the secondary escapeway has been re-routed to the #3 entry, #4 return, brk. 3. A meeting was held with all employee's on changed with the secondary escapeway.

Pillar #2 into 3rd cut between #11 and #12 crosscut, a piece of rock fell on the Joy continous miner. No ventilation was affected, the fall was approx. 4'x10'x15' to heavy for the miner to back up with, the rock was pulled off the miner with a retreaver jack.

12"" waterline broke impeded the escapeway, miners inby were withdrawn until the primary escapeway has been pumped of water.

A roof fall was found in the #3 track entry, the fall extended into #1 and #2 entry. The fall was approx. 6'x140'x60' the affected area in 3 entry has been cleaned up, cribs and arches were built, no equipment involved or ventilation effected.

Employee was changing a floating axle out on shuttle car when the chain he placed on axle tightened catching 3rd digit finger tip on left hand cutting tip off. Employee has returned to work on regular duty.

Accident type, without injuries

During maintenance shift a rockfall was found while checking the fresh waterline in the #4 main return, #3entry, @ #5 xcut. Fall is appr. 20'x30'x6-8'thick, no ventilation affected. Affected area will be timbered & dangered off. The fresh waterline will be rerouted away from affected area. By the time the supervisor was contacted & got outside to call MSHA it was 05:20

Drill steel was hung up, employee tried to pull it out by hand and twisted left wrist.

Struck by flying object

While operating rail buggy, driveshaft broke loose and caused debris to hit right eye, scratching eye. Employee has returned to work with no restrictions.

A roof fall was found in the Perfect Submains, in # 1 entry of the 2nd panel, this fall is approximately 20'x6'x60. Ventilation was not affected no equipment or persons involved.

Employee was using lifting jack on a de-railed mantrip. While letting lifting jack down, bar sprung up hitting employee above left eye, sutures required. Safety talk conducted on safety measures used with lifting jacks. Employee has returned to reg. duty.

Employee complaining of numbness in both hands on 3-7-08. Continued to work. On 5-17-08 he was diagnosed with carpal tunnel in both hands and continued working. On 7-11-08 employee became lost time due to carpal tunnel surgery on both hands.

Employee was working on putting broken belt back together, when he picked up end of belt and pushed forward he felt pain in shoulders. Didn't become lost time till 4/14/08.

A roof fall was found during a weekly exam. Located in the neutral, in #2 entry Brk. 16 on #10 belt, the size of the fall was approx. 20' x 30' x 5' thick. No equip. was involved and ventilation has not been effected. The area has been cribbed and dangered off.

Struck by falling object

Employee was building a stopping when a piece of drawrock fell hitting him on his head and his left shoulder. Employee has a swollen clavicle and has been placed on restricted duty.

Employee was drilling hole in top with drill steel when a piece of rock fell cutting his left forearm, 6 sutures were required, employee has returned to work with no restrictions.

A roof fall was found in the old 019 section, behind #1 bleeder, the last open break at the face. The size of the fall was 125"" long, 8'high,and 17' thick. The area has been danger off and additional roof support will be placed.

Employee was pulling miner cable slack, when he strained his lower back, this accident became reportable on 10-15-07, employee was put on restricted duty from 10-15-07 to 10-22-07, employee has return to regular duty with no restriction.

Fall from machine

Employee was unloading supplies with a battery scoop. As he was getting out of scoop he fell backwards. Employee has been placed on light duty, he has a strain to the lower back.

Struck by flying object

Several pieces of 4' waterline coupled together were being moved when a piece broke apart at the coupling hitting employee in the left leg; he received a fracture to the left fibula.

Employee strained his back on his lower right side, employee continue to work. On 9-6-07 employee became restricted duty due to injury.

A roof fall was found in the #5 entry from crosscut 13-15, the fall is approx. 120' long, the fall is approx. 3' to 7' thick, the fall area was cribbed and dangered off. Additional roof support has been put up in entry 5 & 6 out-by the fall, 8 ft. tension bolts were installed.

Employee was pulling up cable slack from roof bolter, bottom was muddy at the time, employee strained his lower back.

Employee strained his back on his lower right side, employee continue to work. On 8-01-07 employee ceased work complaining of pain in lower back on right side. Injury became reportable on 8-2-07.

Struck by flying object

He was pulling a belt pin that was hung, he struck a hammer with another hammer and a piece of metal broke off and lodged in his right thumb. It was imbedded and was removed at a hospital. He is on restricted duty.

Struck by falling object

The injured employee was struck by a piece of drawrock. He was reaching for a roof bolt when he was stuck on his right arm. His arm was bruised and he was placed on restricted duty for seven days.

Struck against stationary object

The employee was pulling a belt pin out when a piece of metal became lodged in his right thumb. He went to the doctor and had it removed. He retuned to regular job his next scheduled shift.

Employee was working at the power center, he dragged his hand across a piece of metal that had a sharp piece of metal on it. His left ring finger was cut on it. Sutures were required and he returned to his regular job his next scheduled shift.

While putting his starter barrel up a piece of drawrock fell striking him on his left arm. Sutures were required he returned to his regular job.

Inhalation of radiations, caustics, toxic and noxious substances

Employee brought in a statement from NIOSH stating that he has pneumoconiosis. This statement was brought to the company on 08/30/2006.

A roof fall was found on the #2 belt. The fall was located in break 22 on #2. Two rows of bolts were pulled out. The fall was cleaned up and the roof supported. Additional support was added inby and out by the fall area.

Employee was turning a right break when a rib rolled off striking him on the left foot. He continued to work and on 6/20 he went back to the doctor and a simple fracture was found in a bone in his left foot. At that time he was taken off work and the accident became reportable.

Employee was operating a bolter when his glove got caught by the drill steel; his little finger on his left hand was dislocated and also had sutures placed in it. He returned to work on light duty.

Struck by powered moving object

Employee was helping guide the low track fork lift, the operator was trying to pick up a load of resin. When he attempted to back up the low track slid and the fork struck EE's left leg in the knee area. He is on restricted duty. X rays and cat scan were taken no broken bones. He returned to work on 12/15/2005.

Employee was taking a pin out of a scoop bucket when he struck his little finger on his left hand. Six sutures were required.

Employee was cutting insulation off of a wire, his knife slipped and cut his left wrist. He received 7 sutures.

Flash burns (welding)

Employee alleged that his eyes were flashed burned, the accident was not reported to the company. I found out today 10/21/2005 that he went to the doctor. The only reason I found out was we received a medical bill. The employee returned to full duty the next scheduled shift. He was given antibiotics for his eyes.

Employee was moving the miner cable out of the road. He was lifting it when he felt a sharp pain in his back. He continued to work until 8/11/2005. At that time his doctor took him off work.

Employee was helping pull a belt pin out when a small piece of metal got in his Thumb. He went to the Doctor on 7/29/2005 and they found a small piece of metal in his thumb. He returned to work on 7/31/2005 his next sheduled shift. No days lost.

Employee has back problems and other reports have been sent in for the same problem. When he went to the doctor they told him that it was the same back problem that flairs up about every year. He is on light duty for ten days. Employee said he did not know exactly how he hurt it but it was like other times he has had back pain.

The 018 joy miner was caught by a roof fall, it was in the final push in the #2 block when a pocket of rock fell and caught the head of the miner. The fall was about 20 wide by 30 foot long.

Employee was walking across the section to get a tool, when a small piece of rock fell from the top striking him on the left hand. He went to the doctor and to 5 stitches put in the wound. He returned to work his next scheduled shift.

Over-exertion in pulling or pushing objects

Employee was helping adjust the head on the bolter, when he felt pain in the lower part of his back. He continued to work until August 9, 2004. At that time a doctor took him off work for evaluation of his condition. This became reportable on 8/9/2004.

A roof fall occured in the #5 mains, in the #5 entry between crosscut #9 and #10. The fall is approx. 18 ft wide by 45ft long by 4 ft to 5 ft thick. Additional support was added to the area immediately surrounding the fall.

A roof fall was found in the #5 entry in 5 mains. the fall was approx. 20 ft by 20 ft to 2 ft to 5 ft. thick.

Employee was operating a diesel buggy on the track. The buggy hit a broken rail and it threw her forward and her head struck a piece of metal. She got cut below her left eye and had seven sutures. She returned to work the next scheduled shift.

The fall occured in the#7 entry, while taking the first cut out of a right break the roof fell. The fall started at the head of the miner and fell through the intersection. The fall was approx. 30ft by 40ft by 5ft thick. The roof was a busted up shale.

A roof fall was found on 6/1/04 at 2:00am. The fall is located one break inby the #6 drive in the left hand crosscut. The fall did not involve any person or equipment. It is approx. 70ft. long x 20ft wide x 4ft-6ft thick. The fall is in an area of shale and slick rock.

Employee injured his back on 4-21-2004 but continued to work at his regular job until 5-18-04. He took off work and returned to work on 5-24-04. It became reportable 5-18-2004. EE has returned to his regular job.

A mine roof fall occured in the #4 pillar at crosscut 27, while pulling pillars. The main mine roof broke and caught the 12-12 joy miner.

This accident did not become reportable until he was taken off work by the Doctor on 2/11/2004. Tony has complained to me that his back hurts him most of the time and got worse on this date and went to the doctor. This will be filed as repeated trauma, because he 0n 3/11/99 he injured his back, then again on 4/12/00 which required surgery.

THE ROOF FALL OCCURRED APPROXIMATELY 19 BREAKS OUTBY THE FACE. THE FALL OCCURRED IN CROSSCUT #5 AND #6 ENTRY. IT IS APPROXIMATELY 22' BY 20' BY 6' THICK. THE AREA WAS BOLTED WITH 4 FT RESIN BO LTS THE AREAS INBY HAVE BEEN BOLTED WITH 5 FT RESIN BOLTS. CRIBS WERE BUILT AROUND THE FALL. OTHERS WILL BE ADDED IN OTHER AREAS. THE MINE ROOF IS LAMINATED SHALE.

EE IS THE MINER OPERATOR ON 018 SECTION. WHILE OPERATING THE MINER A SHUTTLE CAR WAS PULLING OUT FROM UNDER THE MINER WHEN A BLOCK OF COAL WAS THROWN OUT FROM UNDER THE TIRE. IT STRUCK HIM ABO VE HIS LEFT WRIST, HE HAS A SIMPLE FRACTURE TO ONE BONE IN HIS ARM.

THE FALL OCCURRED IN AN INTERSECTION WHERE THE MINE ROOF WAS LAMINATED SHALE AND SAND STONE ABOVE IT. IT FELL OUT UP TO THE SAND STONE. THE AREA WAS SUPPORTED BY 4 FT RESIN BOLTS. THE AREA WAS CLEANED UP AND REBOLTED WITH 5 FOOT BOLTS. THE SURROUNDING AREAS WERE RESUPPORTED AND THE AREA WILL BE SUPPORTED WITH 5 FT BOLTS UNTIL WE ARE IN AN AREA WITH A MORE STABLE MINE ROOF.

A ROOF FALL WAS FOUND IN THE #5 ENTRY ON 5 MAIN INTAKE. THE FALL WAS APPROX. 40' X 20 BY 6' THICK. THE TOP WAS SHALE AND SUPPORTED BY 6' POINT ANCHOR BOLTS. THE AREA WAS SUPPORTED BY CRIBS TO PREVENT ANY MORE FALLS IN THE AREA.

EE WAS LOOKING AT THE MOTOR ON THE BROOKVILLE MANTRIP WHEN THE LID FELL STRIKING HIS LEFT HAND CUTTING HIS FINGERS ON THAT HAND. SUTURES WERE REQUIRED ON HIS MIDDLE FINGER. HE RETURNED TO WORK HIS NEXT SCHEDULED SHIFT.

EE WAS HELPING REPLACE A BOTTOM BELT ROLLER, WHEN HE FELT A SHARP PAIN IN HIS UPPER BACK. HE WENT TO THE DR. ON 12/9/02 & MISSED THE FOLLOWING 3DAYS. THIS BECAME REPORTABLE ON 12/10/02. HE RET URNED TO WORK ON 12/13/02.

EE WAS WALKING BY THE BELT TAIL PIECE, HE FELL HIS LEFT KNEE STRUCK THE TAIL PIECE. THIS OCCURRED ON 10-24-01 AND NEVER GOT WELL. HE TWISTED IT AGAIN ON 11-1-02 AND WENT BACK TO DR TWICE AND T HE DR TOOK HIM OFF WORK AS OF 11-11-02. THAT IS WHEN IT BECAME REPORTABLE.

EMPLOYEE WAS SETTING A 10 HP PUMP WHEN HE FELT A SHARP PAIN IN HIS STOMACH. HE LATER FOUND THAT HE HAS A RUPTURE. HE HAD OUTPATIENT SURGERY ON 11-4-02 AND RETURNED TO WORK THE FOLLOWING DAY. THAT WAS THE ONLY DAY HE WAS OFF WORK. THAT IS WHEN IT BECAME REPORTABLE.

EE WAS SETTING BITS ON THE LONGWALL SHEAR WHEN HE FELT A SHARP PAIN IN HIS BACK. HE WAS ATTEMPTING TO TURN THE DRUM ON THE SHEAR WHEN HE STARTED HAVING PAIN. THIS IS A LOST TIME ACCIDENT.

THE EMPLOYEE WAS HELPING MAKE A BELT SET UP. HE WAS CARRYING ROLLERS WHEN HE SLIPPED IN MUD AND TWISTED HIS RIGHT KNEE AND ANKLE.

EE WAS ASSISTING IN THE TEAR DOWN AND REMOVAL OF A HEAD DRIVE WHEN HE WAS INJURED. HE WAS CUTTING THE BOLTS OUT OF THE OLD CHUTE AT THE DRIVE WHEN A BOLT SHEARED CAUSING THE CHUTE TO FALL. IT FELL AND STRUCK THE GROUND THEN ROLLED OVER AND HIT EE, FRACTURING A BONE IN HIS LOWER BACK. HE WAS TREATED AND RELEASED FROM THE HOSPITAL WITH A BACK BRACE.

EE WAS OPERATING A BUGGY ON THE TRACK AND STRUCK ANOTHER BUGGY PARKED ON THE TRACK. THE LID OVER THE MOTOR FLEW UP AND CAME DOWN ON HIS MIDDLE FINGER ON HIS LEFT HAND.
