EE was moving loader in the reverse direction and hit the embankment causing an injury to his neck. The EE also lifted or carried (2) 5-gallon containers of oil, straining his neck.
Employee was operating a 10SC shuttle car. He hit a bump in the road and it made him hit his head. This resulted in him chipping his left upper eye tooth.

EE was tramming a mac 8 down the roadway when his right wheel caught on a piece of belt causing the steering wheel to spin and catch his right hand fracturing his right thumb.
EMPLOYEE WAS TRAMMING MANTRIP UP HILL WHEN IT FELL OUT OF POINT, STARTED ROLLING BACKWARDS AND ROLLED OVER HIM. HE WENT TO DOCTOR ON 9/14 AND RETURNED 9/15 WITH RESTRICTION.S
EMPLOYEE WAS PUTTING A PIECE OF WOOD UNDER THE TAILPIECE WHEN THE MINITRAC FORK SLIPPED AND HIT HIS LEFT ARM. WHEN HE GOT HIT WITH THE MINITRAC FORK, HE FELL BACK AND BRUISED HIS RIGHT HAND.
EE WAS GETTING OUT OF SHUTTLE CAR WHEN THE CAR ROLLED AND TWISTED HIS LEG BETWEEN THE CAR AND THE GROUND.
EMPLOYEE WAS SCOOPING OLD WORKS WHERE IT WAS LOW. HE WAS BACKING UP, LOOKED AROUND AND HIT AN INSULATING BOLT IN THE ROOF CAUSING HIM TO STRAIN HIS NECK. DID NOT SEE DOCTOR UNTIL 1/21/02. RTW SLIP FROM DR 1/28/02.
