EE and foreman were moving miner cable. EE fell backwards while holding the cable. He stated he felt pain in his back. This EE had a prior injury to his back in the same area on 11/17/11. This is possibly a continuation of this same injury.

EE was drilling a hole for a roof bolt. As he removed the bottom piece of steel the top piece fell out of the hole striking him on his index finger. This resulted in a small hairline fracture.

The miner was transporting steel jacks on a low trac. As he traveled outby the fire curtain on a medium grade, the steel jacks slid off of the forks, As he was loading the jacks back onto the forks, one of the steel jacks slid off striking his left ring finger.

2 BREAKS OUTBY THE FEEDER AT THE PONY DUSTER- SECTION REFUGE EMPLOYEE WAS CUTTING THE HOSE OFF OF THE DUSTER AND THE UTILITY KNIFE SLIPPED AND CUT HIS RIGHT HAND. REQUIRING STITCHES

Over-exertion in lifting objects

The EE was tramming the continuous miner from the #1 heading to the #5 heading in preparation for the next cut. When he approached the #2 intersection in the last open crosscut, he disengaged the power to the miner & bent over to pick up rope attached to miner cable to place on the tail of the miner. When he went to raise up, he felt sharp in lower back, left hip, & into his leg.

A non-injury roof fall was discovered on the MMU No. 003-0 Section. The roof fall was immediately reported to MSHA hotline. The roof fall is located at CC #7 of the 2-North West mains. The roof fall began in the #5 entry and cont'd. through the #7 CC into the #6 entry. The roof fall is approx. 90-95' l. x 5-15' w. x approx. 6-8' h.

A roof fall was discovered in the No 1 return entry of the #2-northwest mains, between break 63 and break 65, just inby survey station #7905. The roof fall was approximately 30'long, by 15' wide and ranged in thickness from 4 1/2' to 5 1/2'. No injuries resulted from the roof fall, and no ventilation effects were detected.

Employee was operating a 10SC shuttle car. He hit a bump in the road and it made him hit his head. This resulted in him chipping his left upper eye tooth.

Note: on June 27, 2011, EE filed a coal workers pneumoconiosis claim with the Kentucky labor Cabinet. This notice was received by our company on July 18, 2011. There is no an accident associated with this claim or report.

EE was attempting to enter a set of air lock doors. He opened the first door and air pressure caused it ti reclose catching the EE's right forearm.

EE was preparing to bolt 2 left crosscut, as he was swinging the drill head around a section of the rib rolled off and struck his right leg from the knee down. The rib roll broke apart upon impact. This resulted in severe bruising to the EE.

EE was helping to re-build a brattice, the blocks rolled out from the pallet and one of the blocks struck EE's right foot behind steel toe. EE finished shift and went to the Dr. 3/21/11. X-rays did show a hairline fracture in his tarsal bone.

Employee got off mantrip to close airlock doors, when the employee turned around the mantrip was upon him. Pinning the employees left leg between the mantrip and the metal air lock doors. This put pressure on the employees left leg and knee.

EE was helping move miner cable out of roadway in the #5 entry, when they started moving the miner cable caught his feet knocking them out from under him. This caused him to fall on his left hip, hurting his lower back. The EE did not go and see a doctor until 12/17/10, the doctor took him off of work.

Employee was helping to make a belt splice. He was reaching on the belt removing the loose belt nails when the employee's co-worker accidentally hit him on the hand with a hammer. The injury resulted in a broken index finger on the employee's left hand.

On 1/1/10 an unintentional roof fall occurred on 002 Pillar section inby spad #9888. This fall caught the #3 M.R.S. Fall was 5'X15'wX15'l. MSHA issued K order which was modified to allow cleanup and t recover M.R.S. Area was bolted with 10' cable bolts. M.R.S recovered at 7:30am on 11/2/10 and K order was terminated. Area was dangered off and timbered

Employee was moving M.R.S. cables when he caught his left ear on a nail that was sticking out of a roof bolt plate.

The employee was hanging continuous miner cable in the #8 entry when his back started to cramp and hurt.

The employee was using a hammer and chisel to knock a pin out of a belt drive boom. When the pin came out the boom suddenly moved forcing the chisel down into the boom pin hole, pinching the employee's little finger on his left hand between the boom frame and the chisel. This resulted in cutting the employee's finger requiring stitches.

Employee was helping a Strata employee close door on a refuge chamber. When he pushed the door closed, his right knee popped loudly and buckled causing severe pain to his right knee. Note: The doctor did not take the employee off work until 10/18/2010.

Employee was hand loading belt on a trailer. He stated that he felt something ""pop"" in his right leg. Within a few minutes, he said his toes went numb and his leg was hurting.

EE was working on a scoop in the No. 3 right crosscut, when a rib rolled off, pinning him against the scoop.

A non injury unintentional roof fall occurred on the MMU No 1003 section.The roof fall occured during retreat mining. The roof fall was located in the #13 entry 2 crosscuts inby survey station # 8263. The roof fall entrapped the cont. miner and the shuttle car. The roof fall was approx. 4-8' thick X 18' wide X 18' long. MSHA was notified.

Employee was helping bolt a lid on diesel scoop. A piece of draw rock approx. 10"" x 4"" x 1"" fell striking him on his left middle finger.

Contact with hot objects or substances

Employee was helping repairman to change a broken torque shaft on the continuous miner. When He removed the bolts from the cover to remove the torgue shaft, hot oil blew out burning his right hand and fingers. He did not receive inhal medical treament until 2/11/2010. This way only first aid until the beeler broke the blister and cleaned burns on 2/17/2010

Unintentional roof fall occurred on the MMV #003 section during retreat or pillar mining. The roof fall entrapped the continuous miner in the #4 entry two crosscuts inby spad # 3648. Roof fall was reported to MSHA by phone on 02/04/2010.

MMU N/003 section was pillaring on retreat mining in the #4 entry. The entrapment of the miner was 12 crosscut up the 3rd/lt panel off the 2-lt submains. Draw rock fell on the head of the miner causing the entrapment of the machine. (MSHA was notified by the telephone on 12/18/09).

Employee was taking rub rail off miner when it fell down catching his left hand injuring the middle part of the index and middle finger required stiches.

Employee was on off side of beltdrive rockdusting. Accident site was muddy. He slipped and fell backwards. Trying to catch himself with left hand he fell over own SCSR on right side of lower back.

Employee was getting out of shuttle car when he stepped into a hole twisting his ankle and breaking it.

EE was in the shop welding. EE put his gloves on when he felt a sting on his left hand. EE's hand turned red and swollen. EE went home and returned back to work with a bad red streak running up from his hand up to almost his arm pit.

EE was installing bolts on fresh turn in at ZRT break, a piece of rock fell from the top and rib bouncing off the canopy. The rock struck EE on the left shoulder and forearm and slid down his back. This resulted in a fracture of the left radius.

The employee picked up a cardboard box and threw it onto the batteries on the scoop. The box did not go all the way upon the equipment. The employee pushed the box with his right hand and felt something pop in his chest.

EE was bolting when he started drilling a hole a piece of rock broke loose catching his little finger between the rock and the drillpot.

An unplanned roof fall occurred on MMU No. 003 section entrapping the continuous mining machine while or during retreat mining in the No. 3 entry. The roof fall occurred in the rt. run of the coal pillar inby survey station #9086. The roof fall is approximately 8 ft long x 15 ft wide x 2 ft thick. (Note: Roof fall reported to MSHA by phone.)

Miner was stuck in #3 entry. Employee and others hooked a chain to a s/car in an attempt to pull it out. Chain broke and part of it hit employee on left hand requiring stitches.

EE was walking down the #5 entry to the power center, when he hit a stap with his left ear.

Employee was changing out a piece of chain on the pick breaker on the feeder. When he hit the chain with a hammer, a piece broke off striking him in the left arm (bicep area).

EE was making a hammer splice in the belt. He hit the splicing tool with a 41 lb shop hammer and a piece of the hammer broke off, striking him on the right arm, above the elbow.

Machine operator heard repairman say OK & assumed he was clear of the machine. He started tramming backwards & repairman was caught between rib & left ripper ring.
