Along the #9 travel way, he failed to adequately lower himself for clearance and was struck in the right forehead by a cross beam installed across the travel way. The contact caused a laceration which required stitches to close.

The injured was tramming a fork lift on the haul road between the marker portal and the mine office. He ran over a rough place in the road causing him to bounce and strike his head on the canopy of the forklift. The mri report received 8-22-05 confirmed injury.
EE WAS HAULING TWO BUNDLES OF BELT RAILS INTO THE MINE. THE TOP BUNDLE 50 RAILS SLID TO THE SIDE. THE EE ATTEMPTED TO PUSH THE BUNDLE BACK ON TOP AND STRAINED THE MUSCLES IN HIS RIB CAGE, RESU LTING IN A DETACHED MUSCLE FROM HIS RIBS.
