Employee lost his balance when material gave way underneath him while climbing. Employee rolled down the entire face of the stockpile approximately 90 feet before coming to rest.
The employee decided to assist with a crane load which was beyond his normal scope of duties. He was walking backwards with a tagline when he stepped off of the structure contacting a lower level.

EE was assigned to replace a bearing on the lower bend pulley (BP) of Conveyor 22B. Crouching on the catwalk with his back to the midrail he attempted to remove the bearing cover by pulling on it, at which time it slipped from his hands causing him to fall backwards against the midrail. The midrail broke and he fell aproximately 12 ft landing on his left shoulder.
Employee fell from loader step striking head. ***Safety Manager notified our local MSHA Office in Las Vegas, NV within the designated time frame.
Employee was washing windshield on loader and fell off.
During the process of replacing a backstop on conveyor 3, both employee & co-worker began to prep the motor. EE had not placed his safety harness on while on the catwalk. His co-worker loosened the bolt & took the nut off, at that instance the bolt shot out, the reducer rotated forward and struck EE throwing him off the catwalk 10 ft into the chute.
EE was walking with a 5 lb. GPS rover pole in his hand. He looked at the GPS to give directions. He was walking on fallen trees. The bark was smooth, wet and slippery. His foot slipped off and rested on soft and uneven ground 18" to 24" below, causing him to sustain a sprain of his left ankle. This Report was filed on 11/20/06, I called MSHA to inform internet problem.
EE was exiting loader, felt sharp pain in lower back
Employee was stepping into a work boat from shore and slipped. He fell into the boat on his right knee. Employee was taken to the ER and diagnosed with a contusion. Employee was placed on light duty.
Employee tripped and fell while walking in railyard.
EE WAS REPLACING THE HIGH PRESSURE FITTING ON THE AC COMPRESSOR ON A CAT 16H GRADER. ON HIS WAY DOWN THE LADDER HE MISJUDGED HIS LAST FOOT PLACEMENT FALLING BACKWARDS HITTING THE BACK OF HIS HEAD ON THE CONCRETE FLOOR. THIS RESULTED IN A LACERATION ON THE BACK OF HIS HEAD. HE WAS TREATED AND RELEASED FROM THE PALMETTO GENERAL HOSPITAL.
EE was loading AN from a tanker truck to the bins on his bulk truck. While in the process of moving the hose from one bin to another on top of his truck, he slipped on some spilled AN. This resulted in his falling and slicing his left shin about 6" below his knee. He was taken to the hospital and recieved three stitches inside the cut and twelve stitches outside.
Employee fell and slipped on loose dirt/gravel.

Employee was running to turn off conveyor belt when he slipped and fell. The employee received a lower back strain from the fall.
Employee was climbing up the ladder of a Cat 777 Haul unit and her hand slipped on the railing, but she did not fall. Later that evening she complained of back pain. The back pain persisted for several days. On May 24 she asked to see a physician because the back pain had not improved. Doctor reveiw revealed muscle spasm and she was treated with medication and placed on light duty.
Employee was exiting Mack twin screw haul truck and fell when his footing slipped on fuel tank. As the employee fell his left elbow was cut from hitting the rocker panel. Employee was taken to Doctor where his elbow cut was closed with glue instead of suture. Employee returned to work with no restrictions.
Employee was attempting to climb up his loader to start the shift when his lunch bag strap fell off his shoulder. In his attempt to reach for it before it fell he let go of his left hand to catch it. He then lost his balance and twisted his shoulder.
Employee was climbing down from the fines recovery dragline and when he stepped onto the ground from the ladder he twisted his right knee.
While walking on a walkway beside a belt conveyor, he tripped on a piece of rope that is used to brack the conveyor. He never fell. The rope was removed. He broke a metal wire in his knee from an auto accident he was in back in 2001.
Employee had just set brake on a rail car. When he decided to exit the rail car, he jumped off the rail car connection knuckle instead of utilizing the ladder. His right foot got caught on the air hose assembly and he fell, twisting his knee.

EE was fueling his loader. He stepped up on a step that runs along the fuel containment area & grabbed the fuel nozzle. When he stepped off the step, his left foot hit the ground & the ankle rolled inward. EE went to the ground in pain & called his supervisor. He was taken to the hospital. He says the foot just did not hit the ground level.

(Employee quit that same day and did not seek treatment for the injury until August 4th, 2005 at which time he was taken to workers comp.)
Employee getting out of the man lift and slipped on some sand - went to grab himself and according to employee "popped his right shoulder out of socket." Employee stated that this had happened before and he was able to pop back in place, did not want to go to docotr, however called next day and wanted to see a doctor.

ON 3/7/05 EE SAW DOCTOR WHO SAID EE PROBABLY HAD TORN CARTILAGE. ON 3/14/05 WE LEARNED THAT X-RAYS CONFIRMED 2 FRACTURED RIBS.
Employee slipped off the last rung of a ladder and bumped his right knee on the catwalk surface of a conveyor on 2-16-05: (Employee requested medical attention for the first aid incident on 2/23/05, at which point he was taken to workers compensation.)
Employee was stepping off of a 920 Loader. When his right foot hit the ground, he felt a pain in his right knee. His knee continued to hurt throughout the day and he was taken to the doctor. He was placed on light duty and scheduled for further diagnostics of the knee.
Employee was working in the dragline. He was climbing down the shoe ladder and lost his footing falling 4-5 feet to the ground. Landed on his feet, but felt a strain to his lower back. Working restricted duty and receiving medication and physical therapy.
EE TWISTED LEFT KNEE WHILE CLIMBING OUT OF RAILCAR
Area posted DANGER Do not enter unstable ground. Employee walking across to berm to retrieve rope that stabilizes float pump in retaining pond. Settling of the ground caused major cracks on top of berm. Employee heel on edge of crack as he attempted to step over it. Dirt gave way and he fell into the crack up to groin. Complained of pain in wrist, elbow, thigh and abdomin.
