Showing all 20Employee was exiting the float truck bed area, utilizing manufactured steps for egress. Instead of stepping straight down, the employee stepped to the left corner of the bumper step. The employee lost his balance striking his left shin on the left corner of the step bumper, resulting in a laceration that required stitches to correct.
Obstructed area causing Operator to improperly dismount.
While dismounting machine, IW stepped on uneven ground conditions, causing foot to slip.
EE was dismounting machine when his foot slipped causing his leg to hit track pad growser.
Employee did not have proper foot placing and twisted left ankle. In an attempt to break fall, employee injured left hand. Employee suffered sprain/strain to ankle and two fingers. No fractures.
Employee was climbing on to an excavator when his foot slipped off the track. He used his right hand to catch him self and this is when his right hand struck the track and caused a small laceration to his index finger. He recieved two stiches in his right index finger and no medications. Returned to work the same day.
Employee was washing down the ground floor of the washer when he slipped and fell to the concrete floor.
Operator slipped, dirt on steps.
Was crossing over a pipe on the West Side Float Crew, with bolts in both hands, and did not see hole, similar to pothole on other side. As he crossed over pipe, right foot went down into pothole, causing him to lose his balance. As he tried to regain balance to avoid falling, twisted right ankle.
SLIPPED ON WET TRACKS
Strained right ankle. Contributing factor uneven ground twisted ankle!
Employee was climbing onto a bulldozer when he reached for the hand grab and missed it which caused him to fall backwards to the ground.
At time of accident, there was neither unsafe acts or conditions contributing to this injury. (MOUNTING DOZER, FELL AGAINST TRACKS & LACERATED SCALP.)
No mechanical hazards or violations of safety rules were noted as reason for accident. Weather conditions were not a factor. (He lacerated his ring finger dismounting equipment)
Improper 3 pt dismount procedure - no use of handrails.
Employee was stranding in the back of a pickup truck retrieving a flashlight from a tool box when he tripped on materials stored in the truck bed and fell out of the truck.
Employee stepped into a puddle of water that was actually a hole that had been washed out by a leaking underground pipe.
EMPLOYEE STEPPED DOWN FROM COMPACTOR TURNING HIS LEFT LEG CAUSING MUSCLE STRAIN. WET CONDITIONS WERE A CONTRIBUTORY FACTOR.
FELL OFF DOZER.
