While out cleaning windows on loader, foot slipped on icy surface and twisted ankle resulting in a fracture.
Employee was pulling and straightening a large set of tire chains that were laying on the floor. A large hook bar was used to help with this. The hook bar slipped off the chain link which resulted in the employee falling backwards and spinning body around and landing on right knee cap on cement floor.
Employee was stepping out of the back of a pickup, she felt a pop in left leg.
Operator was dismounting fuel truck, facing away from the apparatus, yet maintained 3 points of contact. The operator's feet slipped off of the bottom rung causing his left back side to make contact with the hose reel. It was raining at the time of the incident.
Driller Helper was removing fittings from the station pump with a pipe wrench. The wrench lost bite and the helper landed on his back. He continued working for several days, but decided to see a physician on 10/10/2011.
Employee was dismounting his haul truck, stepped on a rock causing him to twist his right leg and fall to the ground.
Employee was changing drill bits when his foot got caught on drill deck. Employee fell into handrail fracturing two ribs.
Employee was walking across grind deck and tripped on a piece of grading. Employee fell on his knee cutting Left knee.
Worker had finished fueling a Cat 14h motor grader, while dismounting the machine using the steps, his right foot slip off the last step resulting in a sprained right ankle.
Getting into 420 pit van to go to work. Slipped on walkway rubber mat falling backwards hitting right hip and left shoulder. EE injured shoulder on 9/11/09. She reported it to supervisor at that time. She did not feel she needed medical attention. Shoulder did not seem to be getting better. We took her to the doctor on 9/28/09. She was then put on light duty for up to 2 weeks.
Employee was tramming forklift to P&H shovel job at bottom of pit. Employee oversped engine and put fork lift into berm. While dismounting forklift employee fell and cut his head.
On 5/20/09 worker climbed down the ladder of a Hitachi EX1800 Hydraulic shovel in order to check the level of the hydraulic oil in the tank. He turned at the bottom of the steps and twisted his left knee. The worker reported the incident to his foreman but did not seek immediate treatment. The worker asked to see a physician on May 22nd and was diagnosed with a sprained left knee.
A bundle of cathode struck the stairway while the employee was on the second stair thread from the top causing him to sprain his right ankle.
Employee (Water Truck Driver)was next to the water valve at the Kline Water Station. As he stood up to to turn off the water valve, his right foot slipped from underneath him causing him to fall forward striking his left knee on the ground. He sustained a factured tibia.
Employee was installing elec. underground. When he was getting out of trench he twisted his left knee.
Employee was on a power pole when his boot cleat lost grip due to a crack in the wood pole causing him to slip 15 to 20 feet to the ground.
Employee was getting back on dozer after getting fuel. He slipped & fell twisting right knee. Took employee to Doctor. He was put on crutches & brace. He wore for 1 week. He went back to Dr. after MRI. Some scar tissue in his knee & was up to employee if he wanted it operated on. He chose not to have the operation & go back on crew.
While pouring a concrete slab, he slipped in wet cement. As he tried to catch himself from slipping, he struck his right hand against the rebar mat on slab. He continued working rest of day to end of his shift and return to work on 4/7/08.
Employee fell off the truck while unloading a propane bottle from the bed of the vehicle. (Turned injury in on 04/15/2008)

Working on P&H Shovel light guards, slipped on track and twisted knee, took EE to doctor. Over time knee started to hurt. April 28th had MRI. A small tear of the miniscus in right knee was found. Knee was operated on teh 16th. The 17th EE started 7 days off. EE is scheduled back to work on night shift on the 23rd.
Employee was mounting equipment when his hand slipped off the handrail and he jarred his right leg as he fell.
The EE was climbing off 375A dozer. EE was climbing down, slipped off the track of the dozer and caught his right foot on bottom step, bending two toes and dislocating them.
While stepping out of truck, stepped backwards into a hole in the road, sprained knee.
Employee was climbing a 4' ladder. As he applied his weight to the 2nd step, the ladder turned. He fell backwards & landed on his back & shoulder. EE continued working. Did not miss any work. On July 24th, employee complained of back pain not going away. Took him to doctor. X-ray showed fracture at T12-L1. EE is off work now.
Climbing on dozer to get in cab. Got to the top lost balance started to fall backward. He then jumped so not to land on his back. Landed on his feet breaking bone in his left foot.
Climbing up ladder on water tank to see if it was lined up with water island pipe, while coming down ladder he tripped on a hose, twisted ankle and fell landing on exhaust and burned arm.
EE was on grader hand slipped on icy tire. Caused him to loose his balance. Slid down the tire like a slide landed on his feet on the ground and then he tumbled over.
Getting off haul truck, walking to van at shift change. Slipped on ice and twisted knee.
WALKING OUT TO SNOWPLOW AND SLIPPED IN THE SNOW.
WHILE WALKING TO PARKING LOT AT SHIFT CHANGE, SLIPPED ON ICE & HIT HIS HEAD. FOLLOW UP WAS ON 1/06/06, DR. GAVE EMPLOYEE A SHOT IN HIS NECK TO HELP WITH THE PAIN.
While walking to a service truck, the employee slipped on some ice and twisted his knee.
EE STEPPED OFF LADDER OF 793 HAUL TRUCK INTO A HOLE AND STRAINED HIS KNEE. UNEVEN GROUND
Employee tripped on sample catcher. Stepped backwards and left leg went thru hole in deck hitting bit cutting left leg below the knee. (Fourteen stiches).
Dozer operator was standing on the ground waiting to be picked up at the end of shift. He doesn't know if he stepped on a rock or in a hole and twisted his ankle.
EMPLOYEE WAS USING A CHIPPING GUN TO CLEAR ROCK FROM OVER HEAD IN THE SURGE BIN AT THE CRUSHER. MATERIAL FELL ONTO THE EE & HE FELL FROM THE LADDER TWISTING HIS KNEE.
EE WAS TYING LADDER TO THE HEADER AT TAILINGS. THE LADDER SLIPPED AND EE FELL WITH THE LADDER.
Employee bumped right kneecap on access ladder while boarding a haulage truck.
WHILE USING A CHAIN WRENCH, IT BROKE & EMPLOYEE FELL ONTO HIS RT. SHOULDER.
Employee tripped on a lanyard and fell approx. 5' off of a ladder. There was no lost time or restricted duty at this time. Later problems were diagnosed as acute carpal tunnel injury which required surgery.
Employee was dismounting a haulage truck on an inclined ladder. He caught his heel on a step and turned his foot under, falling on it.
As the employee stepped out of the office onto the steps, he lost his balance and fell onto the floor. He extended his right arm to break his fall and dislocated his shoulder.
4100 crane disassembled...Rt pendant lines removed from spreader bar...removing last Lt. line when spreader bar rotated causing emp to lose balance & fall 8" to ground. employee okay, showed supervisor lt foot & ankle...was able to rotate foot w/out pain. refused medical treatment. Drove to Lander, WY Hospital visit & x-ray diagnosed fractured heel.
Hole for drill steel in deck plate of new drill was not equipped with cover for use with mast down. Ee was examining drill steel/bit when he stepped into hole. Ee suffered a twisted ankle and fractured fibula.
Ee was cutting a plate with a track torch when the plate fell and the ee twisted the ankle while getting out of the way.
