Employee was climbing down the steps of the quarry truck. When leaving the last step he jumped and did not let go of the hand rail and pulled a muscle in his arm.
Employee tripped over a door stop and hit his lip on the concrete. Treatment required sutures
Employee slipped on mud and injured his back. He went to the doctor and initially received only first aid treatment and no work restrictions. A follow up appointment was scheduled for 11/3/05. At this time, the employee's back was not getting any better so the doctor prescribed work restrictions (lifting, pushing, and pulling) and physical therapy.
