Struck by falling object

EE was cleaning out a rock and snow jam with a digging bar when a rock fell down and struck a finger which was holding the digging bar. Closed fracture of right ring finger. EE on restricted duty, but never lost any time.

Fall from machine

While stepping on the skid loader emp was power washing, emp slipped and fell (about 18"") to the ground injuring rt shoulder

While walking on haul road ... emp twisted EE's ankle, breaking EE's Fibula.

Fall from ladders

Emp was coming down a 5 foot ladder (used to access haul truck eng compartment). While coming onto the ladder emp put their foot on the 3rd rung. Emp's foot was wet from having (a few minutes ago) gone to the truck for a tool. When emp's foot slipped (wet) the ladder leaned hard to one side and fell over. Emp fell 3 feet to the floor and fractured (hair line) right radius.

Struck by falling object

During Removal of X-bracing, bolts were removed and a weld failed that was holding a 10' piece of 6x6 angle. Angle came down and fractured employee's arm.

Emp. slipped and fell on 6-25 - diagnosed as a sprain. Emp off on 6-26 (working 4 10's) on Saturday 6-27 Dr called emp to see ortho Dr on 6-29. On 6-29 Emp - diagnosed with hairline fracture and told to be out of work. Emp became a lost time accident on 6-29-2020 with a fracture to the tibia of rt leg.

Struck by flying object

EE was observing the maintenance crew work, EE was standing 15' away with all P.P.E. on when a piece of metal the crew was chipping flew at EE striking EE in the tooth.

While holding a bolt in left hand Emp struck bolt with hammer. Emp missed bolt and struck finger on left hand, resulting in 6 stickers.

Struck by falling object

Ground man was cleaning tail screen (using hands) a rock fell and struck rt thumb....resulting in 9 stitches.

Fall from machine

Employee slipped climbing off fuel truck ladder and fell 2.5 feet to the ground. Employee bruised left elbow and swelling soon followed. Employee went to clinic and relieved treatment and physical therapy (2 sessions) Employee was released to full duty with no lost time or restrictions.

The incident occurred during the day time hours. Weather was clear. Did rain in the morning, but was done by the time of the incident.

Employee tripped over guide wire along ramp going into a trailer. Employee fell onto left knee.

Employee was carrying concrete forms from one location on site to another. Felt pain in shoulder and neck. Went to doctor to get evaluated.

Getting off Excavator employee (who is over weight and has arthritis in both knees) fell on EE's bottom and had trouble getting up. Employee used 3 points of contact and when EE's rt foot touched the ground EE released EE's hands and EE's knee went out.

Using an air gun to put a bolt in, employee was holding bolt in place and by accident hit the trigger on the air gun .... spinning the bolt and cutting finger

Struck against stationary object

Working on piece of equipment, rebuilding component and contacted a sharp corner of part I was rebuilding. Incident caused a cut on my left forearm and bled heavily. Picked up tools and applied bandage and proceeded to my hospital at home. There the doctor stitched cut and released me.

Chipping at stuck rocks in crusher with digging bar. Piece of rock flew up over his safety glasses and object got in eye

Employee was moving a 50 gal drum and pinched his rt palm between the floor and the drum....resulting in 3 stitches. Employee returned to full duty...no restrictions

Employee was walking in the parking area heading to his job assignment. He slip and fell on an ice patch and landed on his Right Wrist... Breaking it.

The area was tight and the employee was using a right angle grinder in a pushing manner instead of in a pulling away manner. Employee also was not using a face shield as needed in all duties where flying objects are a hazard. The grinder slipped from his hand and powered back at him, striking him in the nose.

Employee was changing out the grappler to the excavator for the bucket. When he knocked out the pin (with a 5"" long bar) the articulating arm on the grappler fell down and struck the bar. The bar then struck the employee on the right side of his jaw.

Employee was helping to install a blunt tool on a hydraulic hammer when he turned sideways and felt a pain in his back. Employee continued to work for the next 2 days and was unable to work starting 11/29/12

Employee was trimming rubber skirting when the knife blade self adjusted causing it to jump and cut his thumb.

Employee was positioning an I-beam when it shifted and pinched his finger against the structure frame.

Employee was removing rocks that were lodged in a crusher when a rock slid out struck him on the right forearm.

Employee was removing a metal plate from a magnet when it retracted and pinched his finger cutting the tip off.

Employee was using a hammer to reposition a return roller to train the belt. He swung the hammer and missed the roller and his hand went into the roller, causing a fracture and lacerations.

Employee was cutting a tie wrap off with a utility knife when it slipped and cut his thumb. It required 3 sutures.

Ee didn't notify supv of incident until 4/29/09. Allegedly fell to knee w/ pain in back down to knee while loading a shot.

Employee was operating an empty haul truck traveling upgrade at approx. 10 mph and apparently veered to the right and powered up the unit causing the truck to push through the rock berm into a ditch.

Employee was grinding and a piece of steel lodged in his eye. Employee did not report the incident until 6/24/2007.

EMPLOYEE WAS WALKING FROM ONE PIECE OF EQUIPMENT TO ANOTHER AND STEPPED ON A PIECE OF FROZEN CLAY WHICH BROKE AND CAUSED THE EMPLOYEE TO FALL.

Employee slipped on mud and injured his back. He went to the doctor and initially received only first aid treatment and no work restrictions. A follow up appointment was scheduled for 11/3/05. At this time, the employee's back was not getting any better so the doctor prescribed work restrictions (lifting, pushing, and pulling) and physical therapy.
