Slipped and fell

EE was walking across the drag line bench. EE slipped down. EE states that when EE stood up EE felt pain in the groin. EE was seen at DR the next day. No treatment. EE reports no improvement. Dr listed restrictions that could not be met on 04/12/2018. EE missed first shift on 04/16/2018. Follow up with general surgeon on 04/24/2018.

On July 02 2012 employee was climbing up rear steps on a front end loader and slipped and hit his shin. He said he did not need medical treatment. On July 12 2012 employee asked to go to doctor for same injury.

Employee was walking up the steps at the fuel depot to get oil for his Off-Road truck, when his foot did not catch the step all the way, causing him to fall forward. At that time he put his hands down to catch his fall, bending his left wrist backwards. After sending the employee to Doctor's Care, it was determined that he sprained his left wrist.

was getting onto dirt truck, left foot slipped off bottom step and right knee hit bottom step

tripped over wire and hurt left knee on the way to restroom

Was getting onto machine, foot slipped and leg was caught in track and twisted knee.

Employee fell after stepping off roadbase catwalk; looked to see how area around tail section had been washed away to allow unit to turn, as he hopped down with his left foot, ground gave away. He turned to his right trying not to fall on conveyor, he fell to the ground on left side. Said that his left lower back was in pain.

Cleaning the 770 dragline, crossed over catwalk where he was not supposed to be slipped & fell several feet to the ground.

Operator was exiting the dragline to evaluate a new setup. He stepped down off the dragline & turned his ankle in loose soil. The loose soil was from dozer work left behind by first shift clean up of work area.
