Employee was holding rib mesh up with a 6' rod to keep self away from rib while mesh was bolted. EE went to step away and EE's foot was under a loose rock causing EE to fall.
Employee was using a pry bar to pry up a valve bank on a continuous miner when the bar slipped causing the employee to fall. When the employee fell their feet stayed planted to the ground causing EE's body to roll over right ankle causing a fracture to EE's ankle
Employee's right foot became stuck between the relay bar and the toe of shield 140. EE reached over their back towards the next shield and lost balance. Employee fell over fracturing EE's right leg.
An employee was attempting to crossover a pressurized water hose, when the hose burst. The employee then slipped/tripped over a 2" waterline on the ground. This resulted in a sprained/twisted ankle.
Employee arrived to work and entered the bathhouse through the boot wash area. He was ascending the steps. As he stepped onto the concrete floor his foot slipped and right knee buckled. As he fell to the ground he stated he felt his knee cap "pop". He was assisted into the office area for treatment.
Employee was hanging a block curtain by the feeder on the 3rd step of a 6 foot ladder. A service truck was backing up towards the feeder to drop off a spool of co cable and backed into the ladder knocking the ladder and employee to the ground. This resulted in a strain at the time of the incident and now the employee is receiving medical attention.
EE right foot became stuck between the longwall shield foot and the pan line relay bar. EE lost his balance and fell over, fracturing his lower right leg and dislocating his right ankle.
Employee was getting out of the mantrip when he got tripped up trying to get out and he fell on a 6" waterline that was on the ground causing him to hurt his ribs.
Employee was helping to install a mandoor in stopping when he slipped over a crib tie and fell cutting his arm on the door.

I.E. was removing a flow meter from a ladder when he lost his balance causing him to fall off the ladder. This resulted in I.E. spraining his ankle.
I.E. had just completed the process of the ram hydraulic cylinder on the #9 shield on the Longwall face. The investigation indicates that he was trying to install the keeper pin in the cylinder in the rear part of the shield, when he fell and hit his head on the shield. This resulted in I.E. receiving a laceration and minor fractures.
I.E. was on the cable tray working on the shearer. When I.E. was in the process of coming down his right foot slipped down into the cable trough causing I.E. to lose his balance and fall. This resulted in I.E. breaking the tibia bone in his right leg
Employee was walking by the stage loader at the longwall face and twisted his knee. At the time he thought it was nothing so no accident report was filled out. In May it started bothering him so he filed an accident report on 5/4. This injury became reportable on 6/4/15 when he had surgery on his left knee.
Employee was stepping between platform and mainframe on double boom roof bolter. Upon exiting platform his light cord got caught on his bolt tray causing him to stumble and fall. When he fell he tried to catch his self with right arm, feeling a pop in his shoulder
I.E. was on a ladder sealing a stopping. When he started to come down he missed a step. This resulted in I.E. falling backwards off the ladder on his back. This resulted in a back strain.
The employee slipped and fell on a roof bolter. He fell striking his right torso and fractured his 10th rib.
I.E. was going through a mandoor when he tripped and fell on his left arm. This resulted in I.E. tearing a tendon. *I.E. continued to work until 12/15/2014 before taking off.
walking on the refuse dam stepped into a hole and twisted his left knee.

Walking down a walk way on a stacker belt and fell in a hole in the walk way where a piece of walk way was missing and employee had cut his right shin bone. He had no stitches or breaks. Return back to work.
Employee was walking from A sump to A tail when he stepped in a hole and twisted his right ankle.
Hilti salesman is not an employee at M-Class. While the salesman was leaving the prep plant and walking across the parking lot he fell. There was no seen tripping hazards. Possible broken ankle.
While changing oil, employee stepped in hole twisting left knee. First day missed was 11/4/14.
Changing rear end in scoop. Tripped over cribs and hoses on ground. Initially file only. Employee worked up until day of surgery on knee. He has missed no work.
Shoveling along the slope belt when he allegedly lost his footing and fell to the mine floor

While walking to the face of Longwall, employee fell hitting right knee causing laceration that required stitches.

Employee was walking at the bottom of the shaft and slipped, as he slipped he grabbed the screen wire to catch his balance, he strained his left hand ring finger in the mesh, he strained his left ring finger, he continued to work and on December 20th had it operated on, then came back to work.
While climbing into the back of a mantrip he twisted his knee.
Employee was stepping out of the Bobcat and he thought he twisted his ankle, when checked at the hospital he had a broken left foot. He was located at x-cut 24 of the Headgate #3.
Employee was standing on the platform while the ATRS was lowered to put up the last row of bolts, when the ATRS lowered a rock fell ahead of the boom and knocked the operator off the platform. He fell 3 feet to the ground and landed on the piece of rock. He had lacerations to his face and left arm that required stitches. His face had broken bones.
Employee stepped through a mandoor of a stopping and he felt his left knee pop. He continued to work everyday until 5/16/2013 when he had surgery on his left knee to repair ACL damage, the surgeon put staples in his knee. He came back to work on 5/17/2013, but knee surgery was performed on his left knee.
Employee was walking off the longwall face and twisted his left knee.

Injured employee was in process of advancing 2" plastic line for the purpose of pumping material used in constructing pump able cribs. As he was pulling on the line through an area that was wet and muddy he lost his footing, slipped and fell straining his back.

On 11/29, injured was carrying an oxygen bottle. His feet slipped and the bottle came down on his shoulder. He saw a specialist on 12/6 and is currently in physical therapy.
Employee was prying the cap out of a planetary on a roof bolter, he slipped and fell and twisted his left knee. He continued to work until he could not stand the pain any longer, he scheduled meniscus surgery on 11/20/2012, he has been off work since the surgery.
Injured was stepping off Bobcat, missed step and fell landing on his side and lower back on some crib block. He started missing 10/3/12
Employee was inspecting the screen deck on a scalping screen for any missing screen panels, and fell backwards hitting the back of his head, he cut the back of his right side of his scalp, he was taken to the hospital and received stitches to the back of his head.
Employee was loading belt structure and slipped on the trailer, cutting his right shin, causing a deep laceration on his right leg. He went to the hospital and received 11 stitches and returned to work.
Injured was checking water tanks on a ladder and the ladder slipped causing him to fall down the ladder.
