Working on CV 17. When entering the HSI crusher EE slipped and fell, EE tried to catch self and hit EE's shoulder. Then was help by EE's coworkers by making sure EE was ok and called mine supervisor before transporting EE to main office.

Miner reported that he slipped on plastic liner, but that he was OK. Miner AWOL for 3 days before calling in to report that he was seeing a doctor on his own for a strained back. Safety Dept notified 12/09/2011.
Employee was attempting to enter a closed door to the maintenance shop with his hands full of parts. As he was opening the door he placed his foot on the edge of the cement pad in front of the door, lost his balance and strained his back.
