A mechanic was working to remove the final drive of a Cat D10 dozer. EE was repositioning self on the idler frame so that a forklift could be brought in to remove the final drive. EE's hand slipped and EE lost balance and fell backward 42 inches and landed on their back on the track pad (grouser) that had been removed.

A contractor from Kone Cranes was hired to inspect the crane/hoist on the 3rd floor of the prep plant. The contractor stated that they were climbing a ladder to access the crane when the contractor fell from the 4th rung of the ladder. Contractor was complaining of hip, back and head hurting so was accompanied to the ER to get checked out.

Employee was exiting the maintenance truck and as they put weight on their left foot on the step below, they slipped and landed on their right foot, turning their ankle.

The employee had just put a mantrip (buggy) on charge and had taken the cushion into the building for storage and when the EE exited the building EE stated that EE tripped over some cable and fell on shoulder and face. EE stated that EE's neck and shoulder were hurting and chose to drive self to the hospital ER.

Employee was attempting to mount the ladder on a CAT 740B haul truck. His right hand grabbed hold of rail with some grease on it. As he shifted his weight up on ladder his hand slipped and he fell from ladder. His weight came down on his left foot/ankle.

Employee was washing the concrete slab off under the hopper when the primary belt started overloading. The wet pit run starting running over the side of the belt covering his feet in mud. He tried to move and his foot was stuck in the mud. When he twisted his body to move his foot did not move.
