Employee was trying to cut a hole through plastic siding of the prep plant. EE removed EE's gloves to open the knife and forgot to put EE's gloves back on. The knife slipped and cut a finger on EE's other hand. EE was taken to the ER and received 4 stiches to hold the wound closed. EE returned to work the next day.

Mechanic was drilling a hole in EE's mechanic's truck to install a work light. The drill bit "hung" causing the drill to suddenly twist hitting EE in the mouth. This resulted in a laceration. EE was taken to the ER where EE received 3 stitches. EE returned to work the next day.
A mechanic was working to remove the final drive of a Cat D10 dozer. EE was repositioning self on the idler frame so that a forklift could be brought in to remove the final drive. EE's hand slipped and EE lost balance and fell backward 42 inches and landed on their back on the track pad (grouser) that had been removed.
Back Strain caused by a slip.
A contractor from Kone Cranes was hired to inspect the crane/hoist on the 3rd floor of the prep plant. The contractor stated that they were climbing a ladder to access the crane when the contractor fell from the 4th rung of the ladder. Contractor was complaining of hip, back and head hurting so was accompanied to the ER to get checked out.
The mine foreman was assisting in getting the belt drive in the pit unclogged. The belt had somehow become tangled in the drive rollers. They were trying to pull slack in the top belt when the hook failed on the chain hoist (come-a-long). The hoist struck the foreman in the shoulder and the side of the head. EE had a cut that had to have staples and perforated ear drum.
Pulling stuck equipment out, back injury.
Employee was exiting the maintenance truck and as they put weight on their left foot on the step below, they slipped and landed on their right foot, turning their ankle.
The employee had just put a mantrip (buggy) on charge and had taken the cushion into the building for storage and when the EE exited the building EE stated that EE tripped over some cable and fell on shoulder and face. EE stated that EE's neck and shoulder were hurting and chose to drive self to the hospital ER.
A miner was bolting when a piece of draw rock fell between the bolts and the rib hitting EE in the back and knocking EE into the controls. EE was taken to the ER where EE was x-rayed. No broken bones were found. EE later went to a PA that diagnosed EE with a "closed fracture of the twelfth thoracic vertebra". They put EE on "restricted duty".
Employee was using a chain hoist to pull (align) the "low-low" belt structure when the chain hoist broke loose from the anchor point and struck EE in the side of the head.
Employee was assisting in aligning the section belt when the tailpiece broke loose from it's anchor and struck them on the left thigh. EE worked the remainder of the shift and worked the next day. EE work part of the shift of the 22nd but went to clinic who didn't find any injuries. EE went to their family doctor on the 26th and was placed on restricted duty.
An employee was cutting out some old pipe in prep plant using a hand-held grinder with a cutting blade. When the piece of pipe EE was cutting started to fall, EE reached to catch it and EE's left index finger contacted the spinning cutting wheel and lacerated EE's finger.
Employee was on their knees marking centerline and when they got up to a "duck walking" position, their knee "popped" causing some pain. The knee has bothered them intermittently since, so they went to a specialist and had surgery on 9-26-2022. Employee returned to work the next day and is now on restricted work duty on the surface.
Employee was helping to set the tailpiece and was lifting a stiff jack against the mine roof when EE felt the right bicep tear. EE tried to continue working but was in too much pain and had to come out of the mine. EE went to the emergency room and was treated and was referred to a orthopedic specialist. EE came back to work the next day.
The employee parked the 740 Cat Haul Truck on uneven ground. The Employee left the cab to perform some tasks by the wash plant. When getting back into the 740 Haul truck, the step was higher than normal because uneven ground under the ladder, and as EE climbed onto the trucks ladder, EE pulled a muscle in their left leg.
Employee (foreman) was on a personnel carrier while by self. EE said EE backed up and struck EE's head on a brow causing a sore neck. EE finished the shift and didn't want medical attention. EE didn't go to the doctor until weeks after the accident.
The employee was exiting the haul truck and missed the last step with foot. EE tried to catch themselves but fell and hit the ground.
The employee stated that while backing in to be loaded by excavator, the excavator operator lowered the bucket into the bed of the truck causing the truck to abruptly stop. The truck operator stated that's this caused pain in their lower back.
The employee was exiting the haul truck when EE bumped EE's knee on one of the steps.

Employee was loosening a hydraulic line on the skid steer. When the line broke loose employee hit their hands on an oil barrel that had been cut down for draining oil. The sharp edge of the barrel cut employee's hands between the thumb and the index finger on both hands.
The employee was checking the oil in the Komatsu 500 loader as part of the pre-shift inspection and the access door fell on EE's right hand smashing EE's right middle finger tip resulting in a fractured bone.
Driver of a haul truck fell asleep, ran off the road, and turned truck on its side. The driver got a small cut on the head that required one stitch.
A 320C Excavator caught on fire while clearing bush and could not be put out with the use of fire extinguishers. No one was injured.
Employee was tightening the conveyor chain on the MBC, and the grease fitting on the jack blew out, striking EE's left hand.
The employee was attempting to unclog a clogged pipe using an air hose. When the pressure was turned on to the air hose the line whipped around wildly and toggle fitting cut arm.
Bolt steel got hung in top and EE tried to get it out and slipped and hit EE's chest on the drill pot. The bolt steel fell out striking EE's pinky finger.
Loader operator was loading mud and rock when they raised the bucket the rear wheels came off the ground. When the mud slid out of the bucket it threw them against the windshield causing them to cut their head. (Raining and very muddy conditions.)
The truck driver was delivering stoker coal to the load-out to be loaded on rail cars. EE's truck rolled forward into the truck in front of EE. When the truck driver of the truck in front of EE went to investigate, EE found EE leaning over and unconscious. They called for an ambulance and worked with EE until EMS arrived. EE was pronounced dead after arriving at the hospital.
Equipment operator hurt neck and back when operator hit a pothole while operating a front end loader.
EE was moving some metal screens around in the back of pickup truck with the help of a vendor. Doing so, EE's finger got caught between two of them and caused the thin edge of the screen to cut into finger tip. It cut part of the nail so it required 2 stitches.
We are still unsure of what has happened. The employee is under Doctor's care until determination is made.
Employee was spotting a roof bolt when a small piece of draw rock fell and struck ee on the left hand. The result was a hairline fracture of a metacarpal bone on top of the hand.
Employee was descending steps of truck dump when the 2nd to the bottom step broke causing them to hurt their ankle.

The injured employee was assisting in moving a mechanism on the 5th floor of the prep plant when leg was caught between the mechanism and the handrail and broke fibula of left leg.
Employee was assisting in the process of installing waterline when the accident occurred. EE had left hand inside the waterline in an attempt to align it when another employee pushed the waterline inadvertently smashing left pinky finger between the waterline and the rib, causing the tip of the finger to be removed.
Company employee (not mine employee) slipped while exiting excavator and cut right palm on a piece of metal bracing concrete barriers for pond ee was removing silt from.
Employee was cut on the left middle finger by a small piece of drawrock while attempting to install a crib along the roadway.
employee was lifting a slide pan for an MBC when EE strained EE's back. Worked with it until doctor took EE off work on 10-18-16.
Employee was attempting to install an hydraulic pump motor on a Cat scoop when he strained his right shoulder. It was determined on 10-22-15 that medical treatment would be required.
An unplanned roof fall occurred in the crosscut between the no. 4 and no. 5 entries at break #25 on #4 belt. (Approximately 20 breaks outby the face location). The roof fall measured approximately 20' wide, 14' long and 3'-3.5' thick.

Employee was putting fuel in rock truck when he removed the fuel nozzle from tank on the truck, the nozzle and fuel hose slipped out of his hand striking employee in the nose, breaking his nose.
Employee was using a 10 lb. hammer to remove a pin on steering jack on scoop when handle of hammer slipped out of hand striking employee on left thumb breaking bone in thumb.

Operator was travelling in unloaded CAT 740B haul truck. Hit a pot hole in haul road causing him to bounce in seat (whiplash action). Operator has had previous, non-work related issues with neck, which required him to miss time from work. Operator had immediate pain in neck.
