Employee was helping move a 3" suction line. when EE entered the containment area, EE slipped and feet went out from under EE. The employee fell on butt first and rolled over onto right shoulder. EE stated was fine but the next day EE felt sore. On 7-1-16 EE decided that should go in and have shoulder looked at.

Showing all 9A contracted employee was climbing out of a semi-truck when he missed the last step falling to the ground. The contracted employee had his hands full and did not maintain three points of contact while de-boarding.
Injured exited pick-up truck he was operating twisting left knee in the process. Note: Employee stated, on 4-13-15 that the injury was sustained on property. This was the first notification of the injury, provided to Magnetation.
Walking across Plant floor and rolled right ankle. Witness claims there wasn't a slip, trip, or fall. The incident was reported on 7/27/14 to the safety department. Diagnosed with a sprained ankle/foot. Employee received a x ray on 8/28/14 due to residual pain, and was told that a fracture may have occurred. Waiting for recommended CT scan and final doctor notes.

Stepping out of man basket-felt right knee "pop"
Employee was walking from the equipment hill to plant and while doing so stepped in a small hole twisting his ankle causing for a sprained ankle.
He was carrying a 5 gallon bucket of material down a flight of stairs and slipped on the stairs falling on his left low back. He was using three point contact during his travels down the flight of stairs.
EE was walking from the parking lot to the employee locker room and in process of doing so slipped on a slippery spot and fell on his right hip causing some pain.
