Employee was cleaning out the thickener tanks on the south end. While cleaning, employee slipped on mud. Employee fell and twisted left knee.

EE was moving a pallet jack loaded with iron when EE encountered a piece of 1/2 inch by 4 inch plywood covering an expansion joint on the floor. The pallet jack became stuck and as EE pulled hard to get it over the plywood, EE slipped and fell to the floor. EE also noted that there was salt and sand in the area where EE was maneuvering the iron.

Snowing & cold day while outside installing a fire suppression system on a Hydraulic excavator. Employee was wearing insulated bib overalls, as EE was descending the stairs off excavator EE's bibs caught the stairwell causing EE to trip and miss the last 2 stairs landing on the ground on EE's right shoulder, elbow and hand.

Employee tripped and impact caused cut bleeding on little finger on right hand. After employee left for the day the employee decided to go in to get finger looked at and it was found that little finger was fractured. Return to Work report brough back by employee next morning stated to avoid heavy lifting with right hand which will not be an issue with their assigned work tasks.

Employee jumped up on a 3' platform, missed, landing back on ground.

Employee tripped while walking on path, knee made contact with ground causing laceration.

Employee tripped while walking on steel. Chin made contact with steel during fall.

EE states EE was lifting a 3"x3"x8' piece of dunnage that wasn't heavy. EE twisted wrong and felt immediate pain.

Carrying a 1' X 1' X 3/4" piece of steel when the employee tripped and fell, landing on the right hand that the employee was carrying the piece of steel with.

Employee was descending down an A-frame ladder and missed placed footing from 3rd rung to 2nd rung on ladder and lost balance.

Employee stepped on D11R track and opened right side engine compartment door in order to repair air filter housing lid, employee stated that he does not know if he stepped back or lost balance when he opened the door, but he fell between the push arm and the track and landed on the tilt cylinder mount bracket on his left side.

Employee was dismounting the D9 dozer with both hands carrying tools. He stepped onto the push arm & slipped striking both shins on the push arm and subsequently falling to the ground striking his head. He declined first aid medical treatment on 4-9-13. He went to Boon Memorial Hospital on 4-10-13 where he was treated by the E.R. physician and released to return to work 4-17-13.

NOTIFIED OFFICE ON 11/30/12 OF ACCIDENT THAT WAS CLOSING GATE AT SITE ON 11/15/12 AND STEPPED IN A RUT AND KRILLED ANKLE AND KNEE HIT GATE AND HURT LEG

The employee was dismounting the D-75 Cat dozer via the right side push arm. The employee had inadvertently stepped on a rock with his left foot, at which time he fell to the ground and felt pain in his left knee. Employee stated he maintained 3 points of contact while dismounting.

Operator was dismounting WT-10 Caterpillar water truck when he slipped on step and fell backwards striking his right hand on the ground breaking the fifth bone in his hand was treated and released from hospital.

EE fell from a Caterpillar 777-D Rock Truck. It appears that the employee was transferring his dinner bucket from one rock truck to another from the operator's deck to the next rock truck off side fender that was 4ft. away. He over extended himself and slipped and fell between the two parked trucks to the ground. EE has 3 broke ribs and brusing on his left side.

Employee twisted his right knee getting on man trip while checking the deep mine pumps.

Employee lost his footing on the bottom step while attempting to mount a Cat. 992G loader at the beginning of the shift. He fell to the ground, landing on his left knee/shoulder. Diagnosis from Doctor was no broken bones, just bruised.

WEATHER RAINING AND WET GROUND SLIPPED AND FELL

As employee was dismounting his excavator, he lost his footing on the track and fell, landing on his left side (shoulder, back and hip)

EE was getting off the loader. Top step was covered in mud and bottom step was broke. He slipped and scraped his shin on the right leg causing a small laceration on the shin area.

Getting off rt 60 to check out the trk because he had smoke in the cab. He was climbing down the ladder and fell and hit his left knee.

Walking on bench right leg fell in crack.

Walking on drill bench stepped in sink hole, hurt right leg.

As Employee was climbing down the steps of his loader, he lost his footing and fell into the steps, causing a bruise to his right side. He was taken to the hospital where he was x-rayed and released to return to work the following day. On 10/24/11 he returned to the doctor and was diagnosed as having a bone spur in his back and was taken off work until 11/21/11.

As employee was servicing the D11R Dozer, (Co. Unit No. 98), he lost his footing while standing on the push arm and fell backward, landing on his back. This is a distance of approximately 3 feet. Diagnosis is pending.

Employee was walking across office porch and tripped over an elbow and jammed his knee.

As employee started to climb up the ladder to his rock truck, he stepped in a hole causing a sharp pain to his groin area. Diagnosis is pending.

EE was pulling hydro seeding hose over steep hill. He lost his footing and fell to the ground and hit his head as he rolled to the bottom of the ditch.

S/C operator was walking across the section checking his haulage route when employee stepped into a hole turning his right ankle and foot.

EE was opening gate for traffic slipped on gravel as he fell he was still holding on to the gate. EE felt pain in his right shoulder.

Twisted lower back getting out of shuttle car

EE was fueling the truck and stepped onto containment, slipped and twisted his ankle.

Twisted right knee to avoid falling rock.

EE was pre-shifting eq. when he climbed on the truck he slipped hurting his lower back, right hip and upper and lower right leg.

As employee was climbing down off his dozer, he lost his footing and fell backward, causing him to break a bone in his right wrist. He drove himself to the hospital where he was treated and released.

Treating belts with antifreeze, slipped in the snow, falling on left knee.

EE fell on ice while walking across mine yard landing on his back.

Operator had a virus with nausea & diarrhea. The reason this report is late we were trying to find out from the Dr. if ee was off work because of the bump on his head or the virus, we could not get an answer.

EE was preparing to fill tank on water truck. While walking around the truck pulling a chain EE stepped up onto a rock and slipped. Falling to the ground his leg became hung causing him to twist his leg on the way down. At the time ee felt like he could complete his shift and did. EE did go see his doctor a few days later.

Walking up the hill in mine trying to step over shuttle car cable, fell after feet came out from under him falling on right side twisting right knee.

While providing services for Frasure Creek #7 and under the management of Frasure Creek #7 an accident occurred on 6/21/10. The cutter head came unclipped from the power head and resulted in a slip and fall no serious injury only first aid was required and returned back to work on 6/24/10

Employee was standing on cutterhead of Miner when cutterhead came unlatched from powerhead of Miner. When it came loose, it went up in the air approximately 4 feet causing him to fall down on top of cutterhead.

EE was taking fan shroud off 777D when ladder he was on shifted. Felt like he pulled something in the lower left part of his back.
