While employees were preparing for a belt move they noticed that the belt was not level. While trying to level the belt the belt came off onto the belt structure that the employees were standing on causing employee to come off of the structure and hyper extending a knee.

While building a Kennedy overcast, injured was kneeling on the solid, left for overcast crossing, over a 1-2' gap above the completed overcast wall below. The injured lost balance and slid between the gap. Injured stuck left arm out to brace against impact, catching the Kennedy decking before sliding approximately 6' between the wall and solid ground below.

Surface Miner was walking to the forklift. EE stepped over something buried in the snow. When EE did, EE stepped on a belt bottom roller buried in the snow. EE fell back and to the side landing on a roller. This caused broken ribs on the left side.

EE was on Shield #6 and was asked to come into the pan line to help move the mesh out of the way of the bolter. When EE went to go into the panline they lost their balance causing them to fall backwards at which time they hit their head on the relay bar.

Employee fell from the bed of a supply truck while pulling a roll of lay flat waterline off a pallet. Employee braced the fall with right hand and this resulted in a hurt shoulder. Employee was taken to hospital and X-Ray was negative. Employee did not miss any work. It was later found on an MRI that there was damage. Employee had surgery on 12/13/2022 making it a Lost Time injury.

On 5/21/2022 EE slipped and twisted knee. EE was taken to the hospital and X-Ray was negative. EE returned to work and has continued to come to work. An MRI was 6/9/2022 and the results show a strained ACL. EE had ACL surgery 10 years ago and this incident has made it that EE needs Physical Therapy.

Miner claimed miner went to find the crew members in the section & when miner turned a corner slipped in the mud & twisted left knee which caused miner to drop to knees, after that miner's co-workers saw miner & helped miner to the dinner table where they applied ice packs & wrapped the knee. The operator disputes the injury occurred.

Employee got out of the passenger side of a parked mantrip. The mantrip had parked on a piece of wood with the rear tire, leaving the wood leveraged up 4". The employee did not see the wood in walkway and got foot caught under it. This caused the employee to fall with foot still stuck under the wood piece. This resulted in a broken ankle.

While working on the roof bolter, Employee needed some more parts and went to the maintenance skid while walking through a water hole at XC-5 in 1 Right Pines East #2, Employee slipped and felt a sudden pain in lower back. Employee did not miss work until 11/28/2020.

After checking the oil EE stepped up on the left side of UV-024 to check the coolant level. After checking, EE was trying to find a hand hold to help get down off the UV, EE missed grabbing the hole for the radiator cap and fell backwards onto the ground landing on EE's left hand.

Employee was walking past the feeder breaker coming out of the beltline. Employee states employee stumbled and fell onto back and tweaked it. Employee was taken to the hospital for X-Ray. X-Ray was negative and doctor offered RTW full Release. Employee asked for two days off to heal, thus making it a Lost Time Incident.

EE stepped backwards and rolled ankle. It was level ground and EE did not step onto anything. X-Ray showed no injury and EE returned to work. EE had an MRI on their own and saw a doctor on 3/9/2020. Doctor says EE needs surgery. This incident is now reportable and EE will have surgery in two weeks.

Belt man was walking to new location to shovel belt. EE was carrying a shovel in EE's right hand. As EE started down a grade EE got closer to the belt to hold on to dust pipe to sturdy self. The floor was wet closer to the belt and EE slipped and rolled EE's ankle. X-ray showed EE fractured EE's Fibula.

EE was walking to the door to enter the warehouse. They looked over at two other miners while walking, left foot caught the corner of the outside floor mat and they tripped. EE was carrying paperwork and when they fell to the floor they landed on their left elbow.

Employee was walking down the longwall face carrying a rolled up hose. Their right foot slipped forward and they fell backwards injuring the left ankle as they landed on it. EE did not seek medical attention until March 12, 2019. At this time an X-Ray showed a fractured Tibia.

Employee was walking outby on the off side of the belt when they took a step with the right foot and the ankle twisted inward. The resulting injury was a fracture to both the metatarsal #4 and #5 bone.

EE was tying roof mesh to existing roof supports. EE was standing on a ladder in a x-cut by the haulageway. The piece of mesh EE was hanging hung down on one edge and was struck by an on-coming truck which pushed the mesh into the ladder, tipping the ladder over and EE fell to the ground, landing on right arm.

EE was ascending stairs at an overcast when they stumbled near the top and fell striking the left upper arm on the top of the handrail. This resulted in pain to the lower bicep area. EE continued to work regular shifts through 11/19/2017.

EE was mounting a cutter motor on a continuous miner. EE lost balance while stepping off on an 18" high rail causing EE to fall backward. A pry bar EE was using struck EE in the right side as EE fell causing a 1" laceration.

Two employees checking wear plates on crusher chute in sub-zero temperatures. Inspection doors on chute were frozen shut. EE attempting to open door latches with hammer fell from a kneeling position landing on shoulder and head causing fractures and lacerations.

On 11/3/15 the mine operator was informed that the employee could no longer work at full duty due to a partial tear of the ACL in the left knee. On 10/22/15 the employee jumped from a ladder as the ladder was struck by a 28-inch ventilation tube elbow which was pushed into the ladder by a rib roll.

The injured employee is employed by Colorado Lime Company and was injured at Bowie Resources LLC, Bowie No. 2 Mine with MSHA ID 0504591. The injured employee slipped on an icy patch covered by snow and pulled his left hamstring. The treating physician placed him off work. He has returned to work with a full release.
