Employee was stepping off of the track mobile up at the load out building while turning to step away employee's left foot slid out on the rocks causing employee's right leg to twist and buckle. EE fell on right knee with all employee's weight. EE went to doctor the following morning as the pain got worse. EE was put on restrictions for knee pain.

Showing all 4Employee climbed down from a piece of equipment and was walking towards another piece of equipment. While walking employee started to slide on gravel and lose his balance. Employee twisted his torso to regain his balance and felt a pop on his right side just above his waist. Employee felt immediate pain in hip and numbness on the lower left side of his torso just above his hip.
Employee was on the ground walking when he stepped on/or tripped over a rock and lost his balance. He reached out his arm to grab a pipe and catch his fall.
Constructing dry-plant at new sand-mine location. Employee walking across a concrete slab he was looking up while walking attempting to get the attention of co-worker in a boom-lift. While walking he tripped over a piece of rubber conveyor belting that he personally had placed in its location a few minutes prior. He fell and sprained his elbow.
