Employee greasing the plant. When climbing down, EE missed a step & fell, striking the back of EE's leg on an I-beam, causing a bruise. EE declined medical attention on the same day. On 12/02/24, EE requested medical evaluation due to not getting better. Doctor examined EE & placed EE on restricted duty on 12/02/24. Became reportable on 12/02/24.

An employee was attempting to step over a 20" drainpipe to shut off a conveyor belt. The employee was in a rush & in employee's haste to get over the drainpipe, employee lost footing on the uneven ground and injured left leg. Other employees noticed IE on the ground & checked on IE. The supervisor was notified. The injured employee was taken to the hospital to be evaluated.

The employee twisted an ankle when dismounting a forklift. The ground beneath the employee was uneven and caused employee to lose footing. Another employee was in the manlift at the time and saw the injured employee fall to the ground at which point the other employee called nearby employees to assist.

HEO walking up bench when rock that they were on broke away, causing them to fall. Their knee landed on rock & caused cut & scrape injuries. Pain was not bad, so employee continued work. As shift went on, pain increased & knee had swollen. They called supervisor & requested medical attention. HEO taken to ER, X-rays performed & determined that HEO sustained a deep tissue contusion.

Employee was working on a belt at the plant. EE removed the guard, set the guard on the ground behind EE, and continued working. Later, EE turned around to get the replacement roller and tripped over the guard. EE caught self with EE's hands, got up, and continued working. Later, EE began experiencing pain in EE's left shoulder. EE reported the injury on EE's next work day.

As driver got out of the truck, EE dropped the keys and in the process of retrieving them, EE tripped over a pipe bumper rail and fell, causing cuts, abrasions and a sprained thumb.

Employee slipped off the bottom step of the excavator, which caused EE to fall face down on the tracks. Employee was fine at the time, but later decided to seek medical attention at the Emergency Room. Employee sustained contusions to the abdomen and bruising to their ribs.

Employee fractured right leg while helping load equipment onto a low boy trailer. They stepped into a hole where a wood plank was missing. They fell off the trailer and their foot stayed in the hole.

Employee injured right 3rd finger while working on heavy equipment. Per safety manager employee had a conversation with consulting physician and was notified on 5-25-2021 that employee had sustained a fracture and maybe referred to specialist pending follow-up visit on 5-27-2021.

An Employee was walking towards their car, the area was muddy causing them to slip. The employee tried to tighten and tense their body to keep themselves from falling, and started having stiffness. The following day, the employee states they started having pain in the left lower back.
