Struck by... (Not Elsewhere Classified)

An employee was attempting to pry a rock loose in the hopper when the pry bar recoiled & struck EE's safety glasses, resulting in a laceration to the bridge of EE's nose. EE stopped work & was transported to Ward County Hospital's Emergency Room, where EE received five stitches. Following the incident, the employee failed a post-accident drug test & EE's employment was terminated.

Struck by flying object

An employee was standing at the muster point, preparing to leave for the day. A wind gust blew debris into EE's right eye. Employee said safety glasses were worn. Employee attempted to wash out debris. EE still felt grit in the eye & went to Monahans ER. EEs eye was flushed & EE was released with no injury to the eye, a full work release with no restrictions, & an eye prescription.

Fall to the walkway or working surface

An employee was attempting to step over a 20"" drainpipe to shut off a conveyor belt. The employee was in a rush & in employee's haste to get over the drainpipe, employee lost footing on the uneven ground and injured left leg. Other employees noticed IE on the ground & checked on IE. The supervisor was notified. The injured employee was taken to the hospital to be evaluated.

Struck by flying object

Maintenance mechanic was changing out skid plates on log washer. While hammering on the old skid plate below them, a metal fragment flew under their safety glasses and lodged in their eye. Employee reported the incident to their manager. Safety Dept. was notified. The employee was taken to ER and the metal piece was removed.

Fall from machine

Employee slipped off the bottom step of the excavator, which caused EE to fall face down on the tracks. Employee was fine at the time, but later decided to seek medical attention at the Emergency Room. Employee sustained contusions to the abdomen and bruising to their ribs.

Employee punctured left hand above thumb with utility knife. Employee was using utility knife to clean up a wiper when EE accidently stabbed EE's left hand above the thumb. Employee wrapped hand in shirt and went to Monahans ER to be evaluated. Puncture wound required four small stitches. Employee was able to return to work following day.

Fall from machine

Employee injured right 3rd finger while working on heavy equipment. Per safety manager employee had a conversation with consulting physician and was notified on 5-25-2021 that employee had sustained a fracture and maybe referred to specialist pending follow-up visit on 5-27-2021.

Struck by flying object

An employee was attempting to extract a bearing from the filter rock stacker tail pulley. In order to achieve this the employee was hammering the bearing and the second strike to the bearing caused a piece of metal to lodge in the employee's eye. Supervisor was notified immediately and employee was transported to the hospital.

Employee was using pipe wrench and while pulling down on it the pipe wrench lost its bite, and fell from position. In an attempt to catch the wrench from falling to EE's feet, the employee felt shoulder / neck strain.

An employee was using an oxygen acetylene torch to cut steel overhead and during a cleanup cut debris fell and blew into EE's eye due to the wind.

Contact with electrical current

An employee was changing out a fuse on an electrical panel. When EE pulled the fuse out the bottom section, the fuse came out but the top section was stuck in the electrical panel and EE received small shock. The employee was okay at first, but now has neck, back and leg pain as a result of the shock. The employee states was locked and tagged out at the time of the incident.

A mechanic leaned over to pick up some tools and felt a twinge in EE's back. The employee was seen at an occupational clinic.

An employee was getting wood to make a sign from an old pallet and when ee was taking it apart ee swung the hammer and hit left lower leg. EE had a bruise and now has a bump that is red.

An employee was running to the plant to shut down the shaker and feeder as requested, employee hopped from the top of the slope down about 2-4 steps, and landed on their feet when they felt a ""pop"" in the L ankle.

An employee was trying to put a patch on a metal ramp when a 3/4 inch bolt fell 27 inches landing on employee's lip resulting in a scratch inside their mouth and chipping the bottom half their right canine tooth.
