Process supervisor was pulling a water hose when it became caught on something and caused the person to lose balance and fall. A restricted injury occurred.
Warehouse leadman was assisting move ground engagement tools onto a pallet using a lifting magnet. When EE released the magnet and stood up, EE lost balance/slipped and fell, resulting in an injury to shoulder and knee.

While unloading delivery truck, employee pulled on a pallet strap to reposition it. The strap broke, causing the employee to stumble backwards, falling off the rear of the van to the ground below. Employee returned to work but later complained of back pain. EE was examined by a physician on 11/3/16 and placed on restricted duty due to back pain and sprained ligament in back.
Employee stepped into a hole adjacent to a recently poured concrete pad. This action resulted in employee breaking left ankle.
EE stepped in a divot in the ground and knee gave out. Physician ruled as strain. Employee had an MRI on 09/10/2016. Results were sent to us on 9/14/2016 at 3:36pm and degenerative changes found with cyst and small partial tear/fray with Degenerative Join Disease, and Osteoarthritis. Filing because of changes found that make it a recordable if not denied.
Working from a platform while installing concrete forms, employee slipped while picking up EE's pliers causing EE to hit EE's forehead on installed rebar. Employee received 13 sutures to EE's forehead and returned to work. Employee was using fall protection at time of injury.
