Employee was pressure washing the underside of the cone crusher. EE was utilizing a 5' ladder, the ladder shifted as EE was descending the ladder causing EE to lose EE's balance falling backwards off the ladder landing on EE's back. EE's left leg became entangled between the rungs of the ladder. Employee had a back contusion.

ADR operation tripped on a siding frame as EE walked into the plant falling forward, making contact with exposed edge of metal siding with right wrist, resulting in a laceration to the wrist. Laceration required 7 stiches.
