While replacing the coolant fan motor on the Komatsu D375 Dozer, the Miner's foot slipped off the push arm cross-brace causing them to fall towards the push arm where they extended their arms. A metal shard on the push arm sliced the side of their wrist which required 5 stitches to close.

Showing all 9While jumping the starter of the Cat 777C haul truck, the Miner was standing on the tie rod to reach the starter. The Miner slipped on some grease causing the Miner to strike against the truck with Miner's back/side. The Miner suffered three cracked ribs.
While replacing a punch plate deck on the bull-screen, the Miner positioned self above the screen chute at the top of the rubber wear material. As the Miner was maneuvering the screen, EE perceived from EE's view that EE was going to take a step up, but was actually stepping down. The unexpected step down and resulting jolt caused the Miner's knee to twist.
While mounting the Caterpillar 777D Haul Truck, at the beginning of the shift, the Miner's right boot slipped on the suspension ladder causing it to twist and strain.
While dismounting the Komatsu D375 dozer, the Miner's right foot slipped off the outside step on the side frame causing the Miner to fall forward. The Miner put right hand out to brace self and cut hand at the base of the thumb on a bolt guard on the side frame. The Miner went to the hospital where the Miner received 8 stitches.
While moving pieces of drill steel with the Forklift, one of the pieces of steel slid off the side of the forks. The Miner manually slid the steel to remove it and the steel caught the pocket of Miner's coat causing Miner to be dragged to the ground. The Miner struck head on the forks during the fall.
While parked on level ground the Miner dismounted the 777D haul truck to take a break. When stepping down to the ground from the last rung of the access ladder, the Miner felt pain in upper right leg and was unable to walk without pain after completing the step.
After observing the operation from a vantage point adjacent to the northern light plant, the Foreman turned and proceeded toward a vehicle that was to the rear and facing them with the lights on. As they stepped down with the right foot approx. 1' to a different level, encountered a small patch of snow-covered ice causing them to fall on right-rear side.
While bending over to cut the strapping on a pallet, the Miner's footing slipped causing EE to stab self in the leg with a pocket knife.
